Zusammenfassung
Die bakterielle Infektion eines Gelenks stellt wegen der raschen Progredienz, der Gefährdung der Gelenkfunktion und wegen der drohenden generalisierten Sepsis einen Notfall dar. Die Diagnostik beruht auf einer klinischen, sonographischen und radiologischen Untersuchung sowie in der obligaten Gelenkpunktion mit Synoviaanalyse und Keimbestimmung. Die Therapie umfasst eine breite antibiotische Abdeckung des oft multimorbiden Patienten und ein arthroskopisches oder offenes Débridement mit Spülung und Drainage des Gelenks inklusive einer postoperativen Drainagenbehandlung. Die Verwendung von Saug-Spül-Drainagen ist umstritten. Bei unzureichendem Ansprechen des Infekts sollte frühzeitig eine Revisionsoperation indiziert werden. Kurzfristig postoperativ muss mit aktiven und passiven Bewegungsübungen begonnen werden. Falls das Gelenk lasttragend ist, sollte die Extremität für 6 Wochen entlastet werden.
Abstract
Joint infections represent a surgical emergency. For diagnosis, clinical, sonographic, and radiographic examinations are required. Furthermore, puncture of the joint with analysis of joint tap is mandatory. Therapy consists in administration of antibiotics, arthroscopic or open debridement with joint lavage, postoperative drainage and early active and passive mobilization of the joint without weight bearing. Early surgical revision is recommended in cases of insufficient recovery.
