Zusammenfassung
Manche Länder schreiben bei der Einreise bestimmte Impfungen vor, z. B. gegen Gelbfieber, jedoch sind nur wenige Impfzentren von der WHO authorisiert, diese lebenden, attenuierten Viren zu spritzen. Alle anderen Impfungen sind nur freiwillig. Vor einer Reise sollte man den Impfstatus kontrollieren, wobei zunächst die Gültigkeit der Standardimpfungen wie Tetanus, Diphtherie und Poliomyelitis überprüft werden sollte, was dann bei Bedarf wieder aufgefrischt werden sollte. Bei Kindern sollte man auch den Impfstatus bezüglich Mumps/Masern/Röteln feststellen.
Eine aktive Impfung gegen Hepatitis A ist speziell für alle jungen Menschen zu empfehlen, wo man noch nicht damit rechnen kann, dass sie früher schon einmal eine Erkrankung mit diesem Virus durchgemacht haben. Wenn jemand in die gefährdeten Gebiete des Meningitisgürtels von Afrika reist, so sollte man gegen Meningokokken A+C impfen, weil dieser Impfstoff gut wirksam und gut verträglich ist. Alle anderen Impfungen sind nur bedingt indiziert, dies gilt speziell für Typhusimpfung und Choleraimpfung. Neben den Impfungen gehört v. a. eine Beratung über die Malariaprophylaxe zur Vorbereitung. Diese besteht in der Expositionsprophylaxe sowie ggf. in der Chemoprophylaxe. Empfehlungen zur Stand-by-Therapie sind zurückhaltend zu erteilen.
Abstract
Certain vaccinations such as yellow fever are obligatory, at least for some countries. Only a few centers are authorized by the WHO to offer this vaccination with living, attenuated viruses. All other vaccinations are optional. A status analysis of the vaccination record should indicate which of the standard vaccinations, such as tetanus, diphtheria, and poliomyelitis, require booster shots. For children the common vaccinations against mumps, measles, and rubella should be checked.
Active vaccination against hepatitis A is especially recommended for all younger people who most probably have not yet experienced this disease. Individuals traveling to areas of the meningococcal belt in Africa should be vaccinated against meningococci A+C. This vaccine is well tolerated and highly protective. All other vaccinations, including those against cholera and typhoid fever, are of limited value only. Another important concern is malaria prophylaxis when areas with high risk are visited. Both exposition prophylaxis and chemoprophylaxis should be deliberated with the tourist. Standby therapy against malaria is feasible only in certain circumstances. Particular aspects of preventive measures for pregnant women are discussed.
