Zusammenfassung
Endometriose ist eine östrogenabhängige Erkrankung, welche durch das Vorhandensein von ektopem Endometriumgewebe charakterisiert ist. Sie ist eine der häufigsten benignen gynäkologischen Erkrankungen und betrifft etwa 10-15 % der Frauen im reproduktiven Alter und bis zu 50 % aller Frauen mit unerfülltem Kinderwunsch. Neben Infertilität sind Dysmenorrhö, Dyspareunie und chronische Unterbauchschmerzen Leitsymptome der Erkrankung. Ein neu eingeführtes Klassifikationssystem, #ENZIAN, ermöglicht eine systematische, universell einsetzbare Einteilung aller Entitäten dieser heterogenen Erkrankung. Die Operation mit ggf. erforderlicher histologischer Sicherung ist weiterhin der diagnostische Goldstandard und in vielen Fällen auch gleichzeitig die Therapie. Eine Spontankonzeption ist bei vorliegender Endometriose möglich. Zur Verbesserung der Fertilität stehen je nach Ausprägung, Lokalisation und Dauer der Infertilität die operative Therapie, die Insemination mit ovarieller Stimulation oder assistierte Reproduktionstechniken (ART) zur Verfügung.
Abstract
Endometriosis is an estrogen-dependent disease characterized by the presence of ectopic endometrial tissue. It is one of the most common benign gynecological diseases, affecting around 10 to 15% of the female population of reproductive age and up to 50% of all women with infertility. In addition to infertility, dysmenorrhea, dyspareunia, and chronic abdominal pain are the main symptoms of the disease. A newly introduced classification system, #ENZIAN, enables a systematic and universally applicable classification of all entities of this heterogeneous disease. Surgery with potential histological confirmation is still the diagnostic gold standard and, in many cases, also the treatment for the disease. Women with endometriosis can conceive spontaneously. However, options to improve fertility include surgery, insemination with ovarian stimulation, or assisted reproduction techniques (ART), depending on the localization and severity of the disease and the duration of infertility.
