Zusammenfassung
Das Immunsystem spielt im Hinblick auf die Weiterentwicklung zielgerichteter Therapiestrategien auch beim Mammakarzinom eine immer wichtigere Rolle. Insbesondere das triple-negative Mammakarzinom (TNBC) weist im Vergleich zu anderen Subtypen eine höhere Mutationslast auf. Daraus resultiert eine größere Anzahl tumorassoziierter, immunogener Antigene, sodass Immuntherapien speziell für TNBC eine vielversprechende Behandlungsoption darstellen. Das Vorhandensein sowie die Anzahl tumorinfiltrierender Lymphozyten korrelieren positiv mit einer verbesserten Prognose, sowie einem besseren Ansprechen auf Chemotherapie beim TNBC. Eine wichtige Zielstruktur innerhalb des Krebs-Immunzell-Zyklus sind „Immuncheckpoints“. Immuncheckpointinhibitoren (ICPi) blockieren die Interaktion bestimmter Zelloberflächenproteine, die als „Bremsen“ der Immunreaktionen dienen. Jüngste Studien zeigten, dass ICPi das Überleben sowohl beim frühen als auch beim fortgeschrittenen TNBC verbessern. In Kombination mit Chemotherapie sind ICPi bereits für TNBC in der metastasierten Situation zugelassen. Des Weiteren stellen individualisierte Vakzinierungsstrategien gegen tumorassoziierte Neoantigene einen vielversprechenden Ansatz dar. Ein liposomenformulierter intravenöser RNA-Impfstoff, der für verschiedene tumorassoziierte Antigene kodiert, wird derzeit bei TNBC untersucht und führt zu neoantigenspezifischen Immunantworten. Diese neuartigen Therapiestrategien werden die Prognose von Patientinnen mit TNBC verbessern.
Abstract
The immune system plays an increasingly important role in the advancement of targeted therapeutic strategies, including in breast cancer. In particular, triple-negative breast cancer (TNBC) has a higher mutational burden compared to other subtypes. This results in a higher number of tumor-associated immunogenic antigens, making immunotherapies a promising treatment option specifically for TNBC. The presence of tumor-infiltrating lymphocytes correlates positively with improved prognosis, as well as better response to chemotherapy in TNBC. An important target structure within the cancer immune cell cycle are so-called “immune checkpoints”. Immune checkpoint inhibitors (ICPi) block the interaction of certain cell surface proteins that serve as “brakes” on immune responses. Recent studies showed that ICPi improve survival in both early and advanced TNBC. In combination with chemotherapy, ICPi are already approved for TNBC in the metastatic setting. Furthermore, individualized vaccination strategies against tumor-associated neoantigens represent a promising approach. A liposome-formulated intravenous RNA vaccine encoding different tumor-associated antigens is currently being investigated in TNBC, resulting in neoantigen-specific immune responses. These novel therapeutic strategies will improve the prognosis of patients with TNBC.
