Zusammenfassung
Hintergrund
Eine Atonie während einer Sectio caesarea ist eine ernstzunehmende Ursache maternaler Morbidität und Mortalität. Therapiestrategien beinhalten medikamentöse Behandlungen mittels Uterotonika, Uteruskompressionsverfahren sowie interventionelle oder chirurgische Maßnahmen.
Ziel der Arbeit
Vorstellung eines alternativen Konzeptes zur externen Uteruskompression im Falle einer Atonie während einer Sectio caesarea.
Material und Methoden
Alternativ zur herkömmlichen manuellen externen Uteruskompression kann im Fall einer Atonie während einer Sectio caesarea der Uterus intermittierend mittels einer elastischen Binde einbandagiert und damit komprimiert werden.
Ergebnisse
Die Erfahrung mit der Uterusbandage in unserem Zentrum ist durchweg positiv, allerdings liegen nur Einzelfallerfahrungen vor. Komplikationen ergaben sich durch eine Uterusbandagierung nicht. Eine randomisierte, prospektive Studie hierzu existiert jedoch nicht.
Schlussfolgerungen
Die Einbandagierung des Uterus während einer Sectio scheint eine erfolgreiche Zusatzoption in der Behandlung einer Atonie zu sein. Sie kann alternativ bzw. ergänzend zu anderen Maßnahmen herangezogen werden, mit dem Ziel der Vermeidung eines hohen Blutverlustes, der zeitlichen Überbrückung bis zum Ansprechen auf uterustonisierende Medikamente und damit ggf. der Vermeidung einer Hysterektomie.
Abstract
Background
Uterine atony during cesarean delivery is a serious cause of maternal morbidity and mortality. Management strategies include medical treatment with uterotonic agents, compression maneuvers of the uterus, and interventional or surgical procedures.
Objective
Introduction of an alternative concept of external uterine compression in cases of atony during cesarean section.
Materials and methods
As an alternative to the standard manual external compression of the uterus in cases of atony during caesarean section, the uterus can be compressed by wrapping it with an elastic bandage.
Results
The experiences with uterus wrapping in our center are highly positive. No complications were seen after performing uterus wrapping. However, only case series exist and no randomized prospective study is available.
Conclusion
The method of intermittent wrapping of the uterus during caesarean section seems to be a successful additional approach in the management of uterine atony. It may be an alternative or additional treatment option to other procedures in order to avoid high blood loss, gain time until uterotonic drugs take their effect, and thereby perhaps avoid postpartum hysterectomy.
