Zusammenfassung

Woran leiden Patientinnen und Patienten, wenn eine progrediente, inkurable Erkrankungssituation auftritt? Welche Form der Unterstützung braucht es, um solche Belastungssituationen zu lindern oder vorzubeugen? Was leisten in diesem Zusammenhang palliativmedizinische Versorgungsstrukturen, und was sind ihre kennzeichnenden Strukturmerkmale? Warum sollte der Einbezug dieser Strukturen schon stattgefunden haben, ehe schwere und komplexe Probleme aufgetreten sind? Welche Probleme bestehen auch in der Kommunikation über die palliative Erkrankungssituation und über Palliativmedizin, die ein solches frühzeitiges Mit-Daran-Denken erschweren? Der Beitrag erörtert die Grundzüge palliativmedizinischen Handelns, die für eine umfassende Palliativversorgung erforderlichen (Struktur‑)Bedingungen, erläutert die Notwendigkeit und die Evidenz einer frühzeitig im inkurablen Erkrankungsverlauf einzubringenden palliativmedizinischen Unterstützungsperspektive und reflektiert potenzielle Missverständnisse über Aspekte der Palliativmedizin, die allein aufgrund eines divergenten Verständnisses der wichtigsten in der letzten Lebensphase benutzten Begriffe entstehen können.

Abstract

What are the needs and symptoms of patients suffering from incurable, progressive disease? What kind of support may relieve such suffering? What can palliative care services provide for symptom control and comprehensive support, and what features are required for effective relief? Why should aspects of palliative care be brought up early during the course of incurable disease, even before relevant suffering has occurred? What makes communication about the concept of early integration of palliative care and about end-of-life issues in general so difficult? This contribution describes the principles of palliative care, the structural demands, the necessity and evidence for early integration of a palliative care perspective during an incurable course of disease, and reflects potential misunderstandings associated with divergent perceptions of key terms in end-of-life care.
