Zusammenfassung
Blutungsstörungen unter einer menopausalen Hormonsubstitution (MHT) können die Compliance erheblich beeinträchtigen. Deshalb ist auch bei gegebener Indikation für eine MHT auf eine adäquate Therapie von Blutungsstörungen zu achten. Neben organischen Ursachen, wie Myomen und Polypen, spielen auch funktionelle Ursachen (Hormondysbalancen) eine Rolle. Generell müssen prämaligne und maligne Endometriumveränderungen ausgeschlossen werden. Bei der sorgfältigen Anamnese sind auch andere Erkrankungen als mögliche Blutungsursache zu erfassen, wie z. B. ein Hypertonus. Ein wichtiges diagnostisches Element ist die Durchführung einer Vaginalsonographie, wobei auf den adäquaten Messzeitpunkt zu achten ist. Bei Blutungsstörungen und sonographisch suspekten Endometriumbefunden ist zum Ausschluss eines Endometriumkarzinoms eine Hysteroskopie mit histologischer Untersuchung durchzuführen. Dies ist oft auch als ambulante Minihysteroskopie möglich. Die Therapie von Blutungsstörungen unter einer MHT besteht v.a. in einer medikamentösen Behandlung mit einer Dosisvariation oder -anpassung, insbesondere der Gestagenkomponente. Nur in wenigen Fällen ist eine operative Therapie notwendig, die auch minimal-invasiv durch eine operative Hysteroskopie, wie Polyp- oder Myomresektion oder ggf. auch als Endometriumablation durchgeführt werden kann. Das Hauptaugenmerk sollte auf der Prävention von Blutungsstörungen liegen. Blutungsstörungen unter einer MHT erfordern eine individualisierte adäquate Diagnostik und Therapie durch den Gynäkologen.
Abstract
Abnormal uterine bleeding (AUB) during menopausal hormone therapy (MHT) has a negative influence to compliance of MHT. Effective treatment of AUB is necessary if MHT is indicated. Causes of AUB are myoma and polyps and also functional disorders (hormone imbalance). The exclusion of premalignant or malignant endometrial changes is important. Patients should also be asked about other chronic diseases, for instance hypertension. Vaginal sonography is a mandatory part of the diagnostic workup, whereby selecting the time point to measure the endometrial thickness is very important. In patients with AUB and sonographically suspected endometrial findings a hysteroscopic and histological examination must be carried out. This is also possible via outpatient minihysteroscopy. First-line treatment of AUB during MHT is individualized medical treatment with different dosages of hormones especially of progestins. Only in selected cases is surgical treatment useful, which can mostly be performed by an operative hysteroscopy (endometrial ablation, polyp or myoma resection). The main focus should be the prevention of AUB. An individualized diagnostic workup and treatment of AUB during MHT by the gynecologist is necessary.
