Zusammenfassung

Hintergrund

Lungenerkrankungen sind eine wichtige Ursache für Morbidität und Mortalität in der Schwangerschaft.

Ziel

In dieser Übersicht werden die wesentlichen Lungenerkrankungen in der Schwangerschaft anhand aktueller Studien und Leitlinien diskutiert.

Ergebnisse

Asthma bronchiale ist die häufigste chronische Lungenerkrankung bei Schwangeren. Durch optimale Behandlung sind die Risiken für Mutter und Fetus sehr gering. Die ambulant erworbene Pneumonie (CAP) ist die häufigste nichtgeburtliche Infektion Schwangerer mit letalem Ausgang. Die venöse Thromboembolie tritt etwa 5-mal häufiger als bei nichtschwangeren Frauen gleichen Alters auf und ist die häufigste Ursache für Morbidität und Mortalität in der Schwangerschaft. Frauen mit pulmonaler Hypertonie, insbesondere pulmonal-arterieller Hypertonie (PAH), sollte von einer Schwangerschaft abgeraten werden.

Schlussfolgerungen

Die meisten Lungenerkrankungen in der Schwangerschaft sind gut zu behandeln; wichtig sind ein frühzeitiges Erkennen und eine adäquate Therapie.

Abstract

Background

Pulmonary diseases are an important cause of morbidity and mortality in pregnant women.

Objective

The purpose of this review is to cover the main lung diseases that are likely to be encountered in pregnancy.

Results

Asthma is the most common chronic pulmonary disorder during pregnancy in which maternal and fetal risks can be minimized by optimal treatment. In the pregnant patient, pneumonia is the most frequent cause of fatal nonobstetric infection. Some diseases such as pulmonary embolism may be more likely because of the pregnancy itself. Women with pulmonary hypertension, especially pulmonary arterial hypertension, should avoid pregnancy.

Conclusion

Most pulmonary diseases in pregnancy can be treated successfully. Early diagnosis and appropriate therapy are important.
