Zusammenfassung
Vitamin-A-Mangel ist zwar weltweit ein Problem, in ausgeprägter Form in Deutschland jedoch nicht anzutreffen. Eine Unterversorgung ist allerdings auch hier in einigen Risikogruppen wahrscheinlich. Zu diesen zählen vor allem Personen mit streng veganer Kost oder längeren Reduktionsdiäten, Schwangere, besonders mit Mehrlingen und kurzen Geburtsabständen, sowie alte Menschen. Eine Unterversorgung entzieht sich wegen der homöostatischen Kontrolle der Blutwerte so lange der Diagnostik, bis die Leberspeicher weitgehend entleert sind. Doch eine chronische Unterversorgung hat schon vorher Krankheitswert, wie eine Vielzahl von Studien, vor allem aus Entwicklungsländern, belegt. So ist die Infektionsrate von Masernerkrankungen erhöht, und die Erkrankung verläuft meist schwerer. Eine Vitamin-A-Unterversorgung in der Schwangerschaft gefährdet die Lungenreifung des Kindes, erhöht das Infektionsrisiko besonders bei geringem Geburtsgewicht und steigert das Risiko der Mutter für postnatale Komplikationen. Die Unterversorgung lässt sich durch eine gezielte Ernährungsanamnese erfassen.
Abstract
Vitamin A deficiency (VAD) is a global problem especially in developing countries. The severe form of VAD is rarely seen in Germany, but there are risk groups that might have an inadequate supply. In particular, those who follow a strict vegan diet or a weight-reduction diet, women who are pregnant especially after short birth intervals or carrying twins, and finally the elderly. An inadequate supply is difficult to diagnose because of the homoeostatic control of the plasma levels, unless the liver glycogen stores are depleted. However, a chronic low vitamin A intake has clinical consequences, and has been documented in studies from developing countries. In particular, the risk of measles infection and the severity of symptoms increases. A low vitamin A supply during pregnancy has a negative impact on lung development, increases the risk for infections – particularly in newborns with low birth weight – and increases maternal postnatal complications. A low vitamin A supply can be detected through a nutrition questionnaire.
