Zusammenfassung
Mammakarzinom und Osteoporose gehören zu den häufigsten Erkrankungen der Frau und haben Einfluss auf Lebensqualität und Lebenserwartung. Neben vielen anderen Risikofaktoren spielen bei beiden Erkrankungen gewebsspezifische Wirkungen der Östrogene eine entscheidende Rolle. Sie wirken nicht nur auf das Brustdrüsengewebe, sondern beeinflussen auch die Regulation des Knochenstoffwechsels. Dabei stehen direkte rezeptorvermittelte Wirkungen auf Osteoblasten und Osteoklasten sowie indirekte Wirkungen über die Produktion von Zytokinen und weiteren Mediatoren im Vordergrund. Neben dem in der ersten Dekade nach der Menopause am stärksten ausgeprägten Knochenmasseverlust haben auch niedrigste Östrogenserumspiegel in der späten Postmenopause noch Einfluss auf die Knochengesundheit. Im Rahmen der adjuvanten endokrinen Therapie des hormonsensitiven Mammakarzinoms bei postmenopausalen Frauen ist der osteoprotektive Effekt von Tamoxifen hinlänglich belegt. Neue Therapieansätze mit Aromatasehemmern haben in Bezug auf Knochenstoffwechsel, Knochendichte und osteoporosebedingte Frakturen deutlich nachteilige Wirkungen. Aufgrund dieser Zusammenhänge besteht ein wachsendes Interesse an Hintergrundinformationen über Sekundärfolgen adjuvanter, endokriner Therapieprinzipien des Mammakarzinoms. Ziel des Beitrags ist es, einen Überblick über diese Zusammenhänge zu geben und Möglichkeiten der Prävention und Therapie aufzuzeigen.
Abstract
Breast cancer and osteoporosis are two of the most frequent diseases which lead to a decrease of life expectancy and quality of life and estradiol plays a key role in the regulation of bone metabolism as well as in the pathophysiology of breast cancer. In the first decade after onset of the menopause reduction of endogenous estradiol leads to a significant decrease in bone mass. Even in the elderly the very low levels of endogenous estradiol still play an important role in the maintenance of bone health. In the adjuvant endocrine treatment of postmenopausal breast cancer tamoxifen has proven to have a protective effect on bone. In recent years the use of aromatase inhibitors (AI) has significantly increased because of the superior effectiveness with regard to breast cancer outcome; however, a decrease in bone mineral density and an increase in risk of fractures were also shown to be undesirable side effects. This article provides a comprehensive review of the current knowledge with regard to fracture incidence with the use of different AIs as well as prevention and treatment strategies of aromatase-induced bone loss.
