Zusammenfassung
Die Placenta praevia increta birgt ein hohes Risiko für eine schwere postpartale Hämorrhagie. Ein Fertilitätserhalt kann nur bei einem ausgesuchten Kollektiv, mit sorgfältiger antepartaler Diagnosestellung und optimaler Planung an einem Geburtszentrum praktikabel sein. Die Sectio caesarea sollte hier von erfahrenen Chirurgen mit der Möglichkeit einer unmittelbaren intensivmedizinischen Versorgung erfolgen. Postoperativ ist in Abhängigkeit vom Operationsverlauf entweder der Verzicht auf eine weitere Schwangerschaft oder die Entbindung per primärer Sectio caesarea zu besprechen.
Abstract
Placenta praevia increta is a potentially life-threatening condition. Hysterectomy is a common therapeutic intervention but involves the loss of the uterus and the ability to conceive. This raises the question if the responsibility of a hysterectomy sparing operative intervention can be taken after suitable preparation. Incomplete separation of the placenta after delivery involves a high risk of postpartum hemorrhage and subsequent hysterectomy. A caesarean section should be carried out by an experienced surgeon in an institution with immediately available resuscitation and intensive care facilities.
