Zusammenfassung
Eine 38-jährige G3P2 in der 17 + 0 SSW wurde notfallmäßig zu uns verlegt mit vaginalem Flüssigkeitsabgang unter dem Verdacht eines frühen, vorzeitigen Blasensprunges (PPROM) mit septischem Verlauf. Bei der Spiegeleinstellung fand sich rahmiger Ausfluss. Die sonographische Untersuchung gab Hinweis auf einen zervikalen Sitz der Schwangerschaft verbunden mit einer Plazenta praevia partialis. Zudem fand sich eine Invasion plazentärer Anteile in das vordere untere Uterinsegment im Sinne einer Plazenta percreta. Der Fet war zeitgerecht entwickelt, jedoch nicht mehr vital. Laborchemisch wies die Patientin, die elf Jahre zuvor spontan und 15 Jahre zuvor per sectionem entbunden worden ware, eine CRP-Konzentration von 12,8 mg/dl und eine Leukozytose auf. Der diagnostizierte septische Spätabort verbunden mit den sonographisch darstellbaren Plazentationsstörungen waren indikativ für eine Sectiohysterektomie. Intaoperativ konnte der Uterus von der Harnblase distanziert werden. Der postoperative Verlauf gestaltete sich unter i.v.-Therapie mit Clindamycin und Cefuroxim komplikationslos. Aufgrund der steigenden Sectioraten nimmt auch der Risikofaktor „Status nach Sectio caesarea“ stetig zu, sodass jede Indikation zur Sectio caesarea kritisch betrachtet werden sollte.
Abstract
A 38-year-old gravida 3 para 2 presented at 17 + 0 weeks of gestation with the clinical suspicion of a premature rupture of membranes (PROM). The past medical history included a caesarean section 15 years ago and a spontaneous vaginal delivery 11 years ago. On speculum examination a purulent discharge was found. Ultrasonography revealed a cervical pregnancy and a placenta previa partialis with placental invasion of the lower uterine segment. The fetus was adequately developed for gestational age but no vital signs could be detected. Laboratory results showed significantly elevated inflammatory markers (C-reactive protein 12.8 mg/dl and leucocytosis). The diagnosis of septic abortion in combination with a pathological placentation led to the decision to undertake a caesarean hysterectomy. Intraoperatively the uterus could be separated from the urinary bladder. Postoperative treatment included a course of i.v. clindamycin and cefuroxim and further recovery was uneventful. Due to rising caesarean section rates the incidence of placenta previa and percreta is also increasing. To decrease the incidence of pathological placentation the indications for every caesarean section should be considered carefully.
