Unklare Raumforderung im Bereich der Glandula parotis

Zusammenfassung

Eine 74-jährige Frau mit einem drei Jahre zuvor erstdiagnostizierten Mammakarzinom links, das nach Auftreten eines Lokalrezidivs vor einem Jahr mit einer ablativen Therapie behandelt wurde, stellte sich mit einer knotigen Veränderung im Bereich des Kieferwinkels und dem klinischen Bild einer peripheren Fascialisparese vor. Die bildgebende Diagnostik zeigte eine unklare Raumforderung im Bereich der Glandula parotis, die Histologie eine Metastasierung des bekannten Mammakarzinoms. Eine totale Parotidektomie gefolgt von einer lokalen Radiatio hat sich als die Therapie der Wahl zum Erreichen einer lokalen Kontrolle und vor allem zum Erhalt des N. facialis erwiesen. Metastasen in der Glandula parotis sind sehr selten. Bislang wurden in der Literatur nur wenige Fälle einer Metastasierung eines Mammakarzinoms in die Parotis beschrieben.

Abstract

A 74-year-old woman was diagnosed with invasive lobular breast cancer of the left breast 3 years ago. A local recurrence appeared and an ablation was done. The patient now presents with nodular changes in the mandibular angle region and peripheral paresis of the facial nerve. Radiological images revealed an unclear mass in the parotid gland region and histological examination showed metastases of the already known breast cancer. A total parotidectomy followed by local radiation is the first choice therapy for local control of the disease and to preserve the facial nerve. Metastases of the parotid gland are rare and in the literature only a few cases of metastases of breast cancer in the parotid gland have been described.
