Zusammenfassung

Eine 30-jährige IV-Gravida II-Para präsentierte sich mit Unterbauchschmerzen und vaginalen Blutungen nach medikamentöser Interruptio vor 2 Wochen bei damals positivem Chlamydien-Polymerasekettenreaktiontest. Bei normalem Hämoglobin und einer βHCG-Konzentration von noch 2152 mIU/ml erfolgte eine stationäre i.v.-antibiotische Therapie bei Verdacht auf Chlamydien-Adnexitis. Bei akuten Schmerzen mit Kreislaufinstabilität und sonographisch freier Flüssigkeit im Douglas-Raum sowie einem Adnexbefund erfolgte eine notfallmäßige therapeutische Laparoskopie.

Abstract

A 30-year-old gravida IV para II presented with abdominal pain and vaginal bleeding after pregnancy termination with Mifegyne 2 weeks previously and a then positive Chlamydia polymerase chain reaction (PCR) test. With normal hemoglobin and a βHCG (β-human chorionic-gonadotrophin) of still 2,152 mIU/ml, the patient was hospitalized for intravenous antibiotic treatment suspecting Chlamydia adnexitis. Acute abdominal pain, hemodynamic instability with sonographically free fluid in the pouch of Douglas, and an adnexal mass led to an emergency laparoscopy, which revealed a ruptured left tube due to an initially heterotopic tubal pregnancy.
