Zusammenfassung
Aktuelle Daten belegen, dass beide HPV-Impfungen (Gardasil®, Cervarix®) genitale Dysplasien und zahlreiche Konisationen verhindern können. Die Ständige Impfkommission am Robert-Koch-Institut (STIKO) empfiehlt die Impfung gegen HPV für alle Mädchen zwischen 12 und 17 Jahren. Auch wenn für die für Deutschland publizierte statische Kosten-Nutzen-Analyse (Kosten pro gewonnenem Lebensjahr und pro gewonnenem QALY) im Vergleich mit denen anderer europäischer Länder am unteren Rand des Spektrums liegt, ist die HPV-Impfung eine kosteneffektive Maßnahme, vor allem bei zusätzlicher Berücksichtigung von Herdenimmunitätseffekten. Der Beitrag nimmt Stellung zu irreführenden und verunsichernden Behauptungen (u. a. Hypothesen zum Replacement durch andere HPV-Typen), welche Mitursache für die derzeit geringe Akzeptanz vor allem bei Erziehungsberechtigten sind und einen hohen Vermittlungsaufwand für Gynäkologen bedingen. Dargestellt werden weiterhin aktuelle Daten zur Effektivität der Impfung (Entwicklung intraepithelialer Neoplasien, Kreuzprotektion, Wirkung gegen Kondylome, Dauer der Protektion), zu Nebenwirkungen und zur Impfung von Jungen.
Abstract
Recent data show that two papillomavirus (HPV) vaccinations now available in Germany (Gardasil® and Cervarix®) can hinder genital dysplasias and the need for many conisations. The Permanent Vaccine Commission at Germany’s Robert Koch Institute recommends vaccination against HPV for all females between 12 and 17 years of age. Although a published cost:effect analysis (costs per year of life and per quality-adjusted life year) puts Germany on the lower edge among European countries, HPV vaccination still pays off, particularly considering its effects on focus immunity. This article counters misleading and disturbing claims (including hypotheses on replacement by other HPV types) that have much to do with the presently low acceptance by parents and guardians and result in costly selection of gynecologists. Also presented are current data on the vaccine’s effectiveness (development of intraepithelial neoplasias, cross protection, protection from condylomas, duration of protection), side effects, and vaccination of boys.
