Zusammenfassung
Habituelle Aborte können Trauer, Ängste, Stress und Depressionen auslösen. Betroffene Frauen sollten deshalb die Möglichkeit erhalten, sich auszusprechen. Wenn psychische Belastungen ausgeprägter sind oder länger anhalten, sollte eine psychische Behandlung ermöglicht werden. Angst, Depressionen und Stress können auch ursächlich an ungeklärten habituellen Aborten beteiligt sein. Eine mögliche ursächliche psychische Beteiligung ist dennoch im Einzelfall nicht diagnostizierbar, weil auf den psychischen Befund vor einer Schwangerschaft und Fehlgeburt nicht zurückgeschlossen werden kann. Diskutiert wird auch, dass möglicherweise durch psychische Folgen nach Spontanaborten, die sich auf die nächste Schwangerschaft ungünstig auswirken, ein Circulus vitiosus von Fehlgeburten ausgelöst werden könnte. Eine fürsorglich-intensive Schwangerenbetreuung, die sog. Tender Loving Care, ist in prospektiven Studien als effektiv belegt, ihr regulärer Einsatz erscheint gerechtfertigt.
Abstract
Habitual abortions can lead to grief, anxiety, distress and depression. Thus, women should be given the opportunity to articulate their concerns. When psychological burdens get worse or last longer, psychological therapy should be offered. Anxiety, depression and distress can also be involved in the causes of unexplained habitual abortion. Nevertheless, in individual cases, a possible psychological cause cannot be verified, because it is not possible to determine the psychological status before the miscarriage occurred. It should be considered that psychological consequences of a miscarriage affecting the next pregnancy might elicit a vicious circle of miscarriages. Intensive supportive care, so-called tender loving care, has been proved effective in prospective clinical trials and should therefore be provided regularly.
