Zusammenfassung

Fatigue beschreibt einen multisymptomatischen Zustand der körperlichen, kognitiven und emotionalen Erschöpfung. Dieser tritt bei Krebspatienten häufig in Zusammenhang mit oder nach systemischen Therapien oder Bestrahlungen auf, kann aber auch im Zuge der Erkrankung ohne diese entstehen. Der zugrundeliegende Pathomechanismus ist unklar. In der Diagnostik müssen somatische Erkrankungen der Leber, der Niere, des Endokrinums und des Knochenmarks ebenso ausgeschlossen werden wie mit der Krebserkrankung zusammenhängende Ursachen (Schmerz, iatrogene Symptome, Elektrolytstörungen). Therapeutisch steht die Aufklärung des Patienten und seiner Familie über die Erkrankung an erster Stelle. Verschiedene Hilfestellungen sind in der Literatur beschrieben worden und werden im Artikel wiedergegeben. An medikamentösen Therapien stehen Bluttransfusionen, Erythropoese stimulierende Faktoren, psychostimulatorische Medikamente und ggf. Kortikosteroide zur Verfügung.

Abstract

Fatigue is a multisymptomatic condition of physical, cognitive, and emotional exhaustion. With cancer patients it often accompanies or follows systemic therapies or radiation but can also appear independently of them. Its causal pathomechanism is not clear. Diagnosis must exclude somatic disease of the liver, kidneys, endocrine system, and bone marrow and conditions relating to the cause of cancer such as pain, iatrogenic symptoms, and electrolyte disorders. Of primary therapeutic importance is educating the patient and family about the condition, the various approaches for which described in the literature are presented here. Medical therapies include blood transfusions, erythropoiesis stimulating factors, psychostimulatory drugs, and sometimes corticosteroids.
