Zusammenfassung
Die Osteoporose ist die häufigste Knochenkrankheit der Frau und verursacht in Deutschland Kosten in Milliardenhöhe. Frauen erkranken 4- bis 5-mal häufiger an Osteoporose als Männer. Der überwiegende Teil der Osteoporosefälle ist bei der Frau durch den postmenopausalen Abfall der Östrogenkonzentration bedingt. Dies führt dazu, dass nahezu jede 3. Frau nach der Menopause eine osteoporosebedingte Fraktur erleidet. Obwohl mit einem konsequenten Management die Hälfte dieser Frakturen vermieden werden könnte, werden in Deutschland immer noch nur 10% dieser Patientinnen leitliniengerecht behandelt. Nach den neuen Leitlinien des Dachverbands Osteologie (DVO) beruht die Therapieindikation der postmenopausalen Osteoporose auf der Knochendichtemessung mittels DXA-Methode und einer kleinen Zahl klinischer Risikofaktoren. Für eine effektive Behandlung stehen erprobte und einfach applizierbare Medikamente zur Verfügung. Dem Gynäkologen fällt jetzt die Verantwortung zu, die neuen DVO-Leitlinien (2006) mit ihren klaren und wissenschaftlich basierten Empfehlungen bei der postmenopausalen Osteoporose umzusetzen.
Abstract
Osteoporosis is the most frequent bone disease in women and in Germany it generates costs in the millions. Women suffer from osteoporosis four to five times more frequently than men. The majority of cases of osteoporosis in women is due to the postmenopausal decrease in estrogen concentration. As a result almost every third postmenopausal woman suffers from an osteoporotic fracture. Although half of these fractures could be prevented with appropriate management, only 10% of these patients are treated in compliance with guidelines in Germany. According to the new guidelines issued by the Umbrella Organization for Osteology (DVO), the indication for treatment of postmenopausal osteoporosis depends on measurement of bone density with the DXA method and a small number of clinical risk factors. There are well established medical options for effective treatment. It is the responsibility of gynecologists to apply the new (2006) DVO guidelines with their clear-cut and scientifically based recommendations for postmenopausal osteoporosis.
