Zusammenfassung
Das Endometriumkarzinom ist die häufigste maligne Erkrankung des Genitaltraktes der Frau und äußert sich häufig erstmals als postmenopausale Blutung (PMB). Frühzeitig diagnostiziert, besitzt es jedoch eine gute Prognose. Der Altersgipfel liegt zwischen dem 65. und 70. Lebensjahr. Die oft bestehenden Komorbiditäten der betroffenen Patientinnen erfordern die kritische Abwägung prätherapeutischer diagnostischer Verfahren, um potenzielle Übertherapien zu vermeiden. Wesentliche Aufgabe dabei ist, die Indikation zur operativen oder nichtoperativen Behandlung zu stellen. Dabei sind zur Abklärung der PMB die gynäkologische Untersuchung zur Lokalisation einer möglichen Blutung, die transvaginale Sonographie zur Beurteilung des Endometriums und zum Ausschluss weiterer pathologischer Prozesse im kleinen Becken sowie zur histologischen Sicherung die fraktionierte Abrasio unerlässlich.
Abstract
Endometrial cancer is the most common female malignancy, frequently presenting for the first time as postmenopausal bleeding. When diagnosed early it has a good prognosis. Most of them arise in women between the ages of 65 and 70 years. The regularly existing additional disorders of afflicted patients require critical management of pretherapeutic diagnostic procedures in order to avoid potential overtreatment. The main task is the decision between surgical and nonsurgical therapy. To clarify the origin of postmenopausal bleeding, vaginal examination (localization), transvaginal ultrasound (evaluation of endometrium) as well as fractionated curettage (histopathological confirmation) are needed.
