Zusammenfassung
Obstipation ist eines der häufigsten gastroenterologischen Probleme. Ungefähr 12–20% der Erwachsenen in Deutschland leiden unter seltenem Stuhlgang, hartem Stuhl, großen Anstrengungen beim Pressen und manchmal einem Gefühl der unvollständigen Entleerung. Die Prävalenz der Obstipation ist bei Frauen 2- bis 3-mal so hoch wie bei Männern und nimmt im Alter deutlich zu. Die Behandlung wird heute weitgehend durch Selbstdiagnose und Selbstmedikation mit Laxanzien bestimmt. Diese können wie alle anderen Medikamente auch missbraucht werden. Ein Abführmittelmissbrauch liegt vor, wenn die Medikamente trotz fehlender Indikation eingenommen oder bei bestehender Indikation überdosiert werden. Er ist häufig bei jüngeren Frauen festzustellen, die das Abführmittel als Methode zur Gewichtsabnahme ansehen, oder bei Personen mit psychischen Problemen und Essstörungen wie Anorexia nervosa oder Bulimie. Bewusster Abführmittelmissbrauch muss von einem unabsichtlichen Fehlgebrauch unterschieden werden. Letzterer basiert häufig auf einem mangelnden Verständnis der normalen Darmfunktion und den individuellen Stuhlgewohnheiten. Bei den betreffenden Patienten ist Aufklärung hilfreich und notwendig.
Abstract
Constipation is one of the most frequent gastroenterological problems. Approximately 12–20% of the adult population in Germany suffer from infrequent defecation, hard stool, heavy efforts while pressing during defecation and sometimes the sensation of incomplete defecation. Constipation is more common in women than in men and increases in the elderly. The treatment of constipation with laxatives is usually conducted by the patients themselves. Laxatives can be misused like any other pharmaceuticals. We refer of abuse of laxatives if these drugs are taken without any indication or if they are overdosed. Young women often abuse laxatives in order to reduce their weight. Abuse of laxatives is also frequent in patients with anorexia nervosa or bulimia nervosa. Deliberate abuse of laxatives must be distinguished from unintended misuse. Inadequate knowledge of bowel action and individual bowel movement habits often lead to misuse of laxatives. These patients profit from medical education.
