Urogynäkologische Probleme bei Frauen im Alter

Urogynecological problems in elderly women

Zusammenfassung

Der postmenopausale Östrogenmangel führt zu morphologischen, anatomischen und funktionellen Veränderungen am weiblichen Urogenitaltrakt. Die sich daraus ergebenden Probleme sind urogenitale Atrophie, rezidivierende Harnwegsinfektionen, Harndrangsymptomatik, Harninkontinenz und Genitaldeszensus. Die genannten Störungen beeinträchtigen die Qualität des täglichen und sozialen Lebens der Frau. Zusätzlich beeinflussen generalisierte Alterungsprozesse, Begleiterkrankungen und Medikamente das Urogenitalsystem und sind bei der Diagnostik und Therapie zu berücksichtigen. Für viele Fragestellungen sind einfache diagnostische Mittel ausreichend. Konservative Therapieverfahren stehen bei der älteren Patientin im Vordergrund, wobei der lokalen Östrogenisierung eine zentrale Rolle zukommt. Vor operativen Maßnahmen empfiehlt sich eine weiterführende urogynäkologische Diagnostik.

Abstract

Postmenopausal estrogen deficiency leads to morphologic, anatomical and functional changes to the urogenital system. The most commonly resulting problems are urogenital atrophy, recurrent urinary tract infections, urgency, urinary incontinence, and genital prolapse. These pathologic conditions restrict the quality of daily life in the women affected and also their social life, and can lead to a number of secondary problems. In addition to the general processes of aging, comorbidities and concurrernt medications also have effects on the urogenital tract and should be taken into account. In most cases basic diagnostic procedures are sufficient. The therapy is based on conservative approaches in elderly women, and topical estrogen application has a key role. If operative procedures are planned, however, a more extensive urogynecological workup of the patients concerned is advisable.
