Zusammenfassung
Die zumeist postentzündliche Tubenpathologie stellt einen wesentlichen Anteil der weiblichen Sterilitätsfaktoren dar, wobei die rekonstruktive Tubenchirurgie durch den zunehmenden Einsatz von Techniken der assistierten Reproduktion (ART) in den Hintergrund gedrängt wurde. Gerade bei jungen Patientinnen bzw. gering ausgeprägtem Eileiterschaden wäre aber die Operation eine sinnvolle und erfolgversprechende Alternative. Darüber hinaus kann die Tubenchirurgie helfen, die Schwangerschaftschancen nach IVF-Behandlung zu verbessern, wenn eine bestehende Hydrosalpinx beseitigt wird. Zudem kann hierdurch gerade bei mehrfach erfolglos vorbehandelten Frauen eine Alternative zur IVF-Behandlung eröffnet werden.
Insofern sind weiterhin eine moderne Diagnostik und Therapie der tubaren Sterilität unverzichtbar. Erst sie ermöglichen die individuell angepasste, optimale Sterilitätsbehandlung aus dem gesamten therapeutischen Spektrum.
Abstract
Pelvic infections often lead to tubal pathology which represents a major part of female infertility factors. Due to the increasing use of assisted reproductive technology (ART) the role of reconstructive tubal surgery was minimized. Nevertheless, the operative reconstruction and restoration of tubal function can offer a relevant alternative to ART especially in younger women or in case of minor tubal damage. In addition, the treatment of distal tubal occlusion can increase the success rate after IVF treatment. Furthermore, tubal reconstructive surgery might offer an alternative after multiple unsuccessful IVF attempts.
In this respect modern diagnostic procedures and therapy of tubal infertility are still relevant and allow an individually adapted optimal selection of suitable treatment options.
