Zusammenfassung
Das Ovarialkarzinom weist die höchste Mortalität der genitalen Karzinome auf. Meist liegt dies in einer späten Diagnosestellung in fortgeschrittenen Tumorstadien begründet, die durch fehlende Frühsymptome und mangelnde Möglichkeiten von Früherkennung oder Screening bedingt ist. Oft finden sich schon früh Implantationsmetastasen auf dem Peritoneum. Die lymphatische Ausbreitung erfolgt ebenfalls früh, in die pelvinen und dann in die paraaortalen Lymphknoten unabhängig voneinander. Eine hämatogene Aussaat ist dagegen nur selten zu beobachten. So ist schon beim frühen Ovarialkarzinom im Stadium FIGO I und II neben der Hysterektomie und beidseitigen Adnektomie auch die Operation der beiden genannten Lymphknotengruppen erforderlich. Die Therapie des fortgeschrittenen Ovarialkarzinoms (FIGO III und IV) basiert auf der möglichst radikalen Tumorresektion, der sich eine hochwirksame Kombinationschemotherapie anschließt. Die operative Radikalität ist dabei für die Prognose der Patientin mit fortgeschrittenem Ovarialkarzinom von entscheidender Bedeutung.
Abstract
Ovarian carcinoma exhibits the highest rate of mortality among malignant genital tumors. This is usually the result of late diagnosis of the tumor in an advanced stage due to lack of early symptoms and inadequate facilities for early detection or screening. Frequently implantation metastases can be found early on in the peritoneum. Lymphatic invasion also occurs precociously, initially in the pelvic, then in the para-aortic lymph nodes. In contrast, formation of hematogenic metastases is rarely observed. Even in cases of early ovarian carcinoma in FIGO stages I and II, as a rule the necessary management includes hysterectomy and bilateral adnexectomy as well as extirpation of the two lymph node groups already mentioned. Treatment of advanced ovarian carcinoma (FIGO III and IV) is based on the most radical tumor resection possible followed by highly potent combination chemotherapy. The radicality of the surgical procedure is decisively important for the prognosis of patients with advanced ovarian carcinoma.
