Zusammenfassung
Die technischen Fortschritte der Ultraschallgeräte haben den Katalog der Indikationen auf dem Gebiet der Brustdiagnostik erheblich erweitert. Durch zahlreiche Studien wurde dies untermauert. In der Differenzialdiagnostik erreicht der hochauflösende Ultraschall eine Trefferrate von 90–98%. Im präoperativen Staging und in der Erkennung multifokaler Karzinomherde ist der Ultraschall der Mammographie deutlich überlegen. Bei der Abklärung unklarer Herdbefunde wurde der Ultraschall die wichtigste Methode für gezielte interventionelle Eingriffe. Lediglich beim Screening von unsymptomatischen menopausalen Frauen ist bislang allein die Mammographie etabliert. Aber aufgrund mangelnder Sensitivität bei Frauen mit dichtem Parenchym zeigen sich bei prämenopausalen Frauen auch in der Entdeckung okkulter Mammakarzinome große Vorteile für den Ultraschall. Insbesondere in der erweiterten Vorsorge für Risikopatientinnen wird dies in Zukunft einen zunehmenden Stellenwert einnehmen.—Wichtig ist bei der Etablierung des Brustultraschalls jedoch eine gute Standardisierung und Qualitätssicherung.
Abstract
Advances in technology have increased the indications for the use of breast ultrasound. Several studies have confirmed the following indications: High resolution ultrasound allows a sensitivity of 90–98% regarding tumor differentiation. Ultrasound is superior compared with mammography regarding preoperative staging and detection of multifocal lesions. Ultrasound guided interventions are the most reliable method in confirming benign or malignant diagnoses. Only in regards to screening unsymptomatic menopausal women, mammography is still the leading method. But sensitivity of x-ray mammography is limited in women with dense breasts. Therefore ultrasound offers great advantages for detection of small lesions in premenopausal women. In future ultrasound may play an increasing role in early cancer detection of high risk women.—However, in order to establish the wider use of ultrasound technology, standardization and quality control is essential.
