Innovative Koagulations- und Schneideinstrumente in der operativen Gynäkologie

Zusammenfassung

Neben dem seit Jahrzehnten bekannten Einsatz von mono- oder bipolaren Instrumenten hat die medizintechnische Industrie in den letzten Jahren eine Reihe von neuen Koagulations- und Schneidesystemen auf den Markt gebracht, die auf unterschiedlicher energetischer Basis arbeiten. Die z. Zt. in der operativen Gynäkologie am häufigsten verwendeten Systeme sind das monopolare Messer, die bipolare Schere und das LigaSure™ Gefäßversiegelungssystem als Vertreter der Elektrochirurgie, sowie das Harmonic Scalpel^® und der CUSA™, die jeweils ultraschallvermittelt arbeiten. Die Fortschritte gerade in der operativen Laparoskopie sind untrennbar mit der Entwicklung dieser Systeme verbunden. Im Folgenden wird die zugrunde liegende Technik der einzelnen Systeme, deren Anwendungsmöglichkeiten in der operativen Gynäkologie sowie mögliche Vor- und Nachteile beschrieben.

Fazit: Innovative Koagulations- und Schneidesysteme sind nicht zuletzt durch die Verwendung in der minimalinvasiven Chirurgie fest in der modernen operativen Gynäkologie etabliert.

Abstract

In addition to the well known use of mono- and bipolar instruments in electrosurgery, a number of new coagulation and cutting devices, based on variety of physical techniques, have recently been developed. At present, the most often used electrosurgical systems in gynecology are the monopolar scalpel, the bipolar scissors and the LigaSure vessel sealing system. In contrast, the harmonic scalpel and the cavitron ultrasound surgical aspirator work using ultrasound. Recent developments in laparoscopic surgery are closely related to improvements in these coagulating and cutting devices and could hardly have been realizable without them. In the following article, the advantages and disadvantages of these systems and the techniques they are based on are briefly described, including their application in modern gynecology.

Conclusion: not least because of their frequent use in minimal invasive surgery, innovative coagulation and cutting devices have become well established in modern surgical gynecology.
