Zusammenfassung

Symptome der Brust differenzialdiagnostisch zu betrachten, ermöglicht ein rationelles Vorgehen in der Beratung und Therapie der Patientin. Das Hauptsymptom Schmerz kann sowohl eine funktionelle als auch eine morphologisch fassbare Ursache (Milchgangserweiterung, Tumor, Entzündung) haben. Die Knoten-/Tumorbildung erfordert heute (mit Ausnahme der Zysten) eine histologische Abklärung. Die Mamillensekretion lässt vielfach aufgrund der Färbung und Zytologie ihre Ursache erkennen. Galaktographie oder operative Diagnostik können im zweiten Schritt notwendig werden. Die non-puerperale Mastitis muss in manchen Fällen vom inflammatorischen Mammakarzinom abgegrenzt werden. Im Gegensatz zur puerperalen Mastitis verlangt sie eine spezielle Antibiotikagabe oder Operationstechnik. Einer Größenzunahme der Brust liegt oft ein noch nicht geklärter Prozess zugrunde. Hier muss häufig die Indikation zur plastisch-operativen Intervention gestellt werden.

Abstract

Considering breast symptoms from a differential diagnostic point of view allows for the efficient counselling and treatment of the patient. The main symptom, pain, may have a functional or a morphological cause (milk duct dilation, tumor, inflammation). A solid mass requires histological examination, a cyst aspiration cytology. The etiology of nipple discharge can often be inferred from the colour of the fluid. Further diagnostic steps such as galactography and duct excision may be considered necessary. Non-puerperal breast inflammation needs to be differentiated from breast cancer. In contrast to puerperal mastitis, special antibiotics or surgical techniques are required. The reasons for breast enlargement often remain unknown and in most cases plastic surgery is indicated.
