Zusammenfassung
Die im TNM-System von 1997 etablierten prognostischen Faktoren für infiltrativ wachsende Tumoren sind
• Infiltrationstiefe,
• Differenzierungsgrad,
• Lymphknotenstatus,
• Fernmetastasierung.
Von den zusätzlich untersuchten Faktoren haben offensichtlich nur Tumorgröße und Harnstauungsniere prognostische Bedeutung. Im Rahmen der molekularen Marker signalisiert der Verlust der Expression des epithelialen Zell-Zelladhäsionsmoleküls E-cadherin einen klinisch ungünstigen Verlauf [2, 8]. Beim nicht metastasierten Harnblasenkarzinom (T2–4, N0, M0) stellt die radikale Zystektomie die Therapie der Wahl dar. Geht zusätzlich eine neoadjuvante systemische Chemotherapie mit 3 Zyklen M-VAC (Methotrexat, Vinblastin, Adriamycin, Cisplatin) voraus, ist die Überlebensrate signifikant verbessert. Bei Patienten mit lokal fortgeschrittenem Harnblasenkarzinom bietet die adjuvante systemische Chemotherapie mit M-VAC nach Zystektomie und Lymphadenektomie jedoch keinen Überlebensvorteil. In der Therapie des metastasierten Harnblasenkarzinoms verbessern neuere Substanzen wie Gemcitabine oder Taxane nachhaltig die Lebensqualität.
Abstract
The prognostic factors for infiltrating tumors established by the TNM system in 1997 include:
• Depth of infiltration,
• degree of differentiation,
• status of lymph nodes
• distant metastases.
Of the additional factors investigated, only tumor size and hydronephrosis appear to be of prognostic significance. In the scope of molecular markers, the loss of expression of the epithelial cell-cell adhesion molecule E-cadherin signals an unfavorable clinical course. In cases of carcinoma of the urinary bladder without metastases (T2–4, N0, M0), radical cystectomy is the therapy of choice. A preceding neoadjuvant systemic regimen of chemotherapy with three cycles of M-VAC (methotrexate, vinblastine, adriamycin, cisplatin) significantly improves the survival rate. In patients with locally advanced urinary bladder carcinoma, however, adjuvant systemic chemotherapy with M-VAC after cystectomy and lymphadenectomy offers no advantages for survival. Quality of life in patients with metastatic bladder cancer disease is improved by new cytotoxic drugs, i.e. gemcitabine or taxanes.
