Summary
We performed a phase II trial with second-line chemotherapy in patients with refractory, metastatic cancer of the bladder. Second-line therapy consists of 2.6 cycles of mitomycin-C, 5-fluorouracil and folic acid. Two patients suffered from WHO grade III side effects (nephrotoxicity, leukocytopenia). We did not find any complete or partial remission. Four patients had a stable disease; 3.3 months after therapy, patients developed tumor progression with a median survival of 9 months. Four patients reported an improvement in quality of life. Since all patients received supportive care, improvement in quality of life cannot be related to chemotherapy alone. In conclusion, second-line chemotherapy with mitomycin-C, 5-fluorouracil and folic acid has no effect on refractory, metastatic cancer of the bladder and is no alternative for cisplatin and methotrexate-based chemotherapy.
Zusammenfassung
Im Rahmen einer nicht randomisierten Phase-II-Untersuchung sind 15 Patienten mit chemotherapierefraktärem, metastasiertem Harnblasenkarzinom mit einer Second-line-Chemotherapie behandelt worden. Hierbei werden die Substanzen Mitomycin-C, 5-Fluorouracil und Folsäure verabreicht. Die Zytostatika Mitomycin-C und 5-FU weisen keine Kreuzreaktion zu den zuvor eingesetzten Substanzen Methotrexat, Cisplatin, Vinblastin oder Adriamycin auf. Bei 2 Patienten sind Nebenwirkungen WHO-Grad III aufgetreten; dies sind Nephrotoxizität und Hämatotoxizität. Die mittlere Anzahl der verabreichten Zyklen beträgt 2,6. Objektiv meßbare Remissionen im Sinne einer partiellen oder kompletten Remission treten nicht auf; 4 Patienten weisen einen „stable disease“ auf. Im Mittel 3,3 Monate nach Einleitung der Therapie manifestiert sich eine Tumorprogression und die mittlere Überlebensrate beträgt 9 Monate. 4 Patienten geben eine Besserung der subjektiven Befindlichkeit an. Da die Patienten alle eine supportive Begleitmedikation erhalten haben, ist der Einfluß auf die subjektive Befindlichkeit nicht auf die Chemotherapie allein zurückzuführen. Die Second-line-Chemotherapie mit Mitomycin, 5-Fluorouracil und Folsäure hat sich in der Behandlung von Patienten mit progredientem metastasiertem Harnblasenkarzinom als nicht sinnvoll erwiesen und stellt kein alternatives Konzept in der Behandlung refraktärer Karzinome dar.
