Zusammenfassung
Hintergrund
Der Mangel an postmortal gespendeten Organen ist der Hintergrund für mehr oder weniger hohe Raten an Lebendnierenspendetransplantationen weltweit. Seit mindestens 20 Jahren sind auch ABO-Blutgruppen-inkompatible Nierenlebendspendetransplantationen etabliert. Die Gleichwertigkeit der Ergebnisse von ABO-inkompatiblen und -kompatiblen Transplantationen ist in jüngster Zeit angezweifelt worden.
Fragestellung
Im Sinne einer kritischen Reflexion des eigenen Nierentransplantationsprogramms waren wir an dem Vergleich ABO-inkompatibler mit ABO-kompatiblen Lebendnierenspendetransplantationen interessiert.
Material und Methode
Es wurde eine retrospektive Analyse der Langzeitergebnisse aller Lebendnierenspendetransplantationen unseres Zentrums seit der Durchführung erster ABO-inkompatibler Transplantationen 2005 bis einschließlich 2022 durchgeführt.
Ergebnisse
Zwischen 2005 und 2022 haben wir 1099 Nierentransplantationen nach Lebendnierenspende durchgeführt. Darunter waren 241 ABO-inkompatible Transplantationen. Das Transplantatüberleben war nach ABO-inkompatibler Spende signifikant geringer als nach ABO-kompatibler Spende. Dieser Effekt setzte sich aus einer gesteigerten Mortalität der Empfänger v. a. in der Frühphase und einer verringerten Langlebigkeit der Transplantate zusammen.
Schlussfolgerungen
Durch die Einbeziehung von ABO-inkompatiblen Paaren für Lebendnierenspendetransplantationen in Crossover-Programme können sowohl die medizinischen Ergebnisse verbessert als auch Kosten gesenkt werden.
Abstract
Background
The lack of postmortem donated organs is the background to varyingly high rates of living-donor kidney transplants worldwide. ABO blood group-incompatible living-donor kidney transplants have also been established for at least 20 years. The equivalence of the results of ABO-incompatible and ABO-compatible transplants has recently been questioned.
Objective
In the sense of a critical reflection of our own kidney transplant program, we were interested in comparing ABO-incompatible with ABO-compatible living-donor kidney transplants.
Materials and methods
A retrospective analysis of the long-term outcomes of all living-donor kidney transplants performed at our center since the first ABO-incompatible transplants were performed in 2005 up to and including 2022 was performed.
Results
Between 2005 and 2022, 1099 living kidney transplants were performed at the authors’ center. Among them were 241 ABO-incompatible transplants. Transplant survival was significantly lower after ABO-incompatible donation than after ABO-compatible donation. This effect consisted of an increased mortality of the recipients, especially in the early phase, and a reduced longevity of the grafts.
Conclusion
Including ABO-incompatible pairs for living-donor kidney transplants in crossover programs can improve medical outcomes and reduce costs.
