Zusammenfassung
Hintergrund
Durch die FDA-Warnungen (Food and Drug Administration) wurde in vielen Ländern, wie z. B. Frankreich, England, USA, Kanada, Australien und Neuseeland, der Einsatz von alloplastischem Material in der Deszensuschirurgie gesetzlich ausgesetzt, in anderen Ländern wie den Niederlanden und Schweden eingeschränkt. In Deutschland empfehlen die Fachgesellschaften die Umsetzung des SCENIHR-Gutachtens (Scientific Committee on Emerging and Newly Identified Health Risks), wonach nur beim Rezidiv- bzw. komplexen Prolaps der Einsatz von transvaginalen Netzen gerechtfertigt ist.
Fragestellung
Welche etablierten und innovativen Operationsmethoden ohne alloplastisches Material stehen aktuell zur Deszensuskorrektur zur Verfügung?
Methoden
Es erfolgte eine Literaturrecherche über bereits etablierte, leitliniengerechte Standardoperationen, sowie innovative Neuansätze.
Ergebnisse
Die vaginale Korrektur des vorderen Kompartiments hat mit der vorderen Scheidenplastik ein etabliertes Verfahren, jedoch mit einer hohen Rezidivrate. Als Neuansatz ist die zweischichtige Faszienrekonstruktion zu nennen, die bisher nach 19 Monaten ein besseres Outcome aufweist. Zur vaginalen Korrektur des mittleren Kompartiments zählen die Sakrouterinligamentfixation und die vaginale sakrospinale Fixation. Neue innovative Ansätze für das mittlere Kompartiment sind die laparoskopische unilaterale Fadenhysteropexie, sowie die Nutzung der Semitendinosussehne im Sinne einer Pektopexie bzw. Sakropexie, jedoch stehen hier noch Langzeitdaten aus. Zur Korrektur des hinteren Kompartiments ist die vaginale hintere Scheidenplastik mit guten Erfolgsraten die Operation der Wahl.
Schlussfolgerung
Etablierte operative Verfahren erleben eine Renaissance, zudem zeigen neue innovative Ideen zur Optimierung des Outcomes bereits gute, aber noch fehlende langfristige Studienergebnisse.
Abstract
Background
In many countries, such as France, England, USA, Canada, Australia, and New Zealand, alloplastic material in prolapse surgery has been paused due to the US Food and Drug Administration (FDA) warning, and restricted in other countries like the Netherlands and Sweden. For Europe and thus Germany, the SCENIHR report allows alloplastic material to be used for prolapse repair after recurrence and in other special situations.
Question
Which established and innovative prolapse surgeries without alloplastic material are currently available?
Methods
A literature search was carried out on established, guideline-compliant pelvic floor surgeries without alloplastic material as well as innovative new approaches.
Results
An established procedure for a defect in the anterior compartment is anterior colporrhaphy, which is associated with a high recurrence rate. The double-layered anterior colporrhaphy is a new approach and so far is associated with an improved 19-month outcome. Apical pelvic organ prolapse can be corrected by sacrouterine ligament fixation and vaginal sacrospinous fixation. New innovative techniques include laparoscopic unilateral pectineal suspension and the use of the semitendinosus tendon autograft to perform pectopexy or sacropexy. However, long-term data are still pending. In case of a posterior vaginal wall prolapse, posterior colporrhaphy is the therapy of choice and is associated with good success rates.
Conclusion
Well-known surgical procedures with native tissue are experiencing a renaissance and new, innovative surgical approaches with good postoperative results are being developed. However, long-term studies are still necessary.
