Zusammenfassung
Die Hodensonographie mit hochauflösenden Schallköpfen und zusätzlicher Darstellung der Vaskularisation in der farbkodierten Sonographie ist die Methode der Wahl zur Detektion und Charakterisierung von Raumforderungen des Hodens. Im vergangenen Jahrzehnt ist die Hodensonographie durch die Möglichkeiten der Kontrastmittelsonographie („contrast enhanced ultrasound“, CEUS) weiter verfeinert und verbessert worden.
Insbesondere bei der Beurteilung von farbduplexsonographisch schwierig zu beurteilenden Vaskularisationsstörungen (inkomplette Torsion, segmentale Perfusionsausfälle, Abszesse) oder nicht eindeutigen Herdbefunden des Hodens kann CEUS hilfreich sein.
Grundgedanke beim Einsatz von CEUS in der Skrotalsonographie ist dabei die Möglichkeit, zwischen avaskulären, hypo- und hypervaskularisierten Prozessen zu unterscheiden; dabei wird der fehlende Nachweis einer Perfusion in aller Regel als Zeichen der Benignität gewertet. Der Nachweis einer Vaskularisation spricht für eine Neoplasie, aber nicht zwangsläufig für Malignität (Beispiel: Leydig-Zell-Tumoren). Besonders wertvoll ist die CEUS bei der korrekten Einordnung von kleinen (< 1,5 cm messenden) Raumforderungen.
Dieser Übersichtsartikel fasst den aktuellen Stand über Indikationen, Implikationen und Grenzen des Einsatzes von CEUS bei Hodenraumforderungen zusammen.
Abstract
Scrotal ultrasound with high-resolution transducers and additional display of vascularisation in colour-coded ultrasonography is the method of choice for the detection and characterisation of testicular tumors. Within the last decade, testicular ultrasonography has been further refined and improved by the possibilities of contrast-enhanced ultrasound (CEUS).
CEUS may be especially helpful in the assessment of vascularisation disorders (e.g., incomplete torsion, segmental perfusion disorders, abscesses) or testicular foci remaining equivocal even after colour-coded ultrasonography.
The basic idea in the application of CEUS in scrotal sonography is the possibility to differentiate between avascular, hypo- and hypervascular findings, with a lack of perfusion generally considered as a sign of benignity. Vice versa, detection of vascularisation speaks for neoplasia but not necessarily for malignity (e.g., Leydig cell tumors). CEUS is especially valuable in the correct classification of small (<1.5 cm) testicular foci.
This review summarizes the present knowledge on indications, implications and limitations of the application of CEUS in testicular tumors.
