Zusammenfassung
Zwei hauptsächliche Entstehungswege des Peniskarzinoms sind gesichert: HPV-assoziierte (HPV: humane Papillomaviren) Tumoren und HPV-unabhängige Karzinogenesen bei chronischen Dermatosen. Folglich könnte die HPV-Präventivimpfung u. a. zu den zeitgenössischen Maßnahmen der Peniskarzinomprävention zählen. Die dabei erreichte B‑Zell-vermittelte Immunität gegen die HPV-Hüllproteine erwies sich als effektiver Schutz vor zukünftigen HPV-Infektionen. Um im Gegensatz dazu vorhandene HPV-Infektionen oder HPV-assoziierte Tumoren zu behandeln, bedarf es einer antigenspezifischen T‑Zell-Immunität. Bis heute sind Krebsfrüherkennung und Behandlung von Krebsvorstufen zur Peniskarzinomprävention im Deutschen Gesundheitssystem nicht etabliert und die dringlich erwarteten therapeutischen HPV-Impfstoffe sind weiterhin eine Zukunftsvision. In diesem Artikel beschäftigen wir uns mit möglichen Präventionsstrategien auf verschiedenen Ebenen der HPV-abhängigen Peniskarzinomentstehung.
Abstract
Two major pathways of penile carcinogenesis are known: human papillomavirus (HPV)-induced penile cancer and HPV-negative cancers associated with chronic dermatoses. Therefore, modern measures for prevention of penile cancer may for example include prophylactic HPV vaccination. The resulting B‑cell-mediated immunity to HPV capsid proteins is effective protection against future HPV infections. Contrarily when treating existing HPV infections or HPV-associated cancers an antigen-specific T‑cell immunity is necessary. To date, screening and treatment of precancerous lesions to prevent penile cancer are not established in the German health care program and the highly expected therapeutic HPV vaccines are still on the horizon. In this article, we focus on possible strategies to prevent HPV-related penile cancer on different levels of carcinogenesis.
