Zusammenfassung
Hintergrund
Die Bedeutung der Lebendnierenspende hat in den letzten Jahren zugenommen. Durch den zunehmenden Organmangel und den steigenden Bedarf verlängert sich für den Patienten automatisch die Wartezeit, was für den Dialysepatienten einen dramatischen Anstieg von Morbidität und Mortalität zur Folge hat. Im Falle einer Lebendspende kann dieses Risiko verringert werden oder aber durch eine präemptive Spende sogar nahezu minimiert werden.
Ziel
Im Vergleich zur postmortalen Transplantation bietet die Lebendspende, begünstigt durch kurze Ischämiezeiten und optimal logistische Voraussetzungen Vorteile hinsichtlich der Transplantatfunktion und des Transplantatüberlebens. Das kürzlich entscheidend überarbeitete Transplantationsgesetzt dient neben dem Schutz des Spenders auch zur rechtlichen Absicherung und dazu Missbrauchsfälle zu verhindern. Die Bundesärztekammer erarbeitet derzeit auf Grundlage der Empfehlungen des Amsterdamer Forums allgemeingültige Richtlinien zur Lebendspendevorbereitung. Dabei finden nicht nur diagnostische Standards Berücksichtigung, sondern auch technische Aspekte für die Organentnahme und die Kriterien für die Auswahl des Organs. Ziel dieser Bestrebungen ist es, dem Spender durch eine einheitliche Vorgehensweise die größtmögliche Sicherheit zu gewährleisten.
Schlussfolgerung
Die Lebendnierenspende kann derzeit als die medizinisch effektivste Form der Nierentransplantation angesehen werden und stellt höchste Anforderungen an die beteiligten Ärzte.
Abstract
Background
The living kidney donation has become increasingly important in recent years. Because of the decreasing number of postmortem donors, there has been a dramatic increase in morbidity and mortality due to the long waiting times for patients on dialysis. By timely living donation after dialysis entry or even preemptively, this can be avoided.
Aim
In addition, the living donor has better graft function and better graft survival which is due to the predictability of the donation, the optimal conditioning of donor and recipient, and the short ischemia time. To protect the donor, to provide legal protection, and to avoid abuse, the German legislature reacted with the Transplantation Act and its amendment. The recent recommendations for donor evaluation from the Amsterdam Forum have been used by the Federal Medical Council to revise current guidelines and guidelines for living donation will be drawn up.
Conclusion
The focus of these efforts is standardization of the procedure and protection of the kidney donor. This is also reflected in the recommendations for organ removal technique and the selection of the organ to be used for kidney donation.
