Zusammenfassung
Hintergrund
Die Weiterbildungsassistenten der Urologischen Kliniken in Deutschland befinden sich derzeit in einer schwierigen Situation: Neben Problemen mit der Umsetzung des Arbeitszeitgesetzes und überbordender Bürokratie steht v. a. die Qualität der operativen Ausbildung im Fokus ihres Interesses. Es wird leider vielfach angenommen, dass operative Ausbildung die Operationszeit unnötig verlängert und das operative Ergebnis gefährden kann. Zudem lehnen viele Patienten es ab, „Lehrobjekt“ zu sein.
Fragestellung
Am Beispiel der transurethralen Prostataresektion als typische Ausbildungsoperation verglichen wir retrospektiv die peri- und postoperativen Ergebnisse der Patienten, die durch einen beaufsichtigten Weiterbildungsassistenten operiert wurden mit denen, welche die Operation durch einen Facharzt erhielten.
Patienten und Methoden
Alle evaluierten Patienten erhielten eine transurethrale monopolare Elektroresektion der Prostata bei symptomatischer benigner Prostatahyperplasie. Sämtliche Parameter wurden retrospektiv evaluiert. Das mittlere Follow-up zur Erfassung der funktionellen Ergebnisse und Spätkomplikationen betrug 70 Monate.
Ergebnisse
Beide Patientengruppen unterschieden sich lediglich im resezierten Prostatagewicht zu Gunsten der Facharztgruppe. Sämtliche übrigen perioperativen Parameter inklusive der Operationszeit und der Frühkomplikationsrate unterschieden sich nicht. Ebenfalls bestand keine Differenz im Hinblick auf funktionelle Ergebnisse und Langzeitkomplikationen.
Schlussfolgerung
Die transurethrale Elektroresektion der Prostata ist auch unter Ausbildungsbedingungen ein sicheres und effektives Verfahren zur Therapie der benignen Prostatahyperplasie.
Abstract
Background
German residents are becoming increasingly more discontent with their situation. Besides unfriendly working hours and growing bureaucracy the lack of surgical education is considered as causative. Surgical education is assumed to be time-consuming and might impair operative results. Moreover, a number of patients refuse to be misused as a teaching tool.
Aim of the study
The aim of this study was to compare operative procedures (transurethral resection of the prostate) performed by residents and consultants.
Patients and methods
All patients underwent monopolar transurethral electroresection of the prostate for symptomatic benign enlargement. All data were collected retrospectively. Mean follow-up to collect functional results and late complications was 70 months.
Results
Both groups did not differ significantly in perioperative and postoperative results except for the weight of resected tissue in favor of the consultants group. Functional results and late complications were comparable in both groups.
Conclusions
Transurethral resection of the prostate performed by residents is a safe and effective procedure.
