Zusammenfassung

Vorrangiges Ziel der operativen Hypospadiekorrektur ist die Rekonstruktion der primären Fehlbildung zu einem normalen Penis. Bei heute großzügiger Indikationsstellung wird die Operation um den 1. Geburtstag durchgeführt. Der Eingriff erfolgt in definierten operativen Schritten in Abhängigkeit der vorgegebenen Hypospadiekomponenten. Aktueller Trend ist es, die Harnröhre aus der Urethralplatte zu rekonstruieren und bei ausgeprägten proximalen Hypospadien mit hochgradiger Peniskrümmung zweizeitig vorzugehen. Das perioperative Management inkludiert moderne Kinderanästhesieverfahren kombiniert mit Regionalanästhesie, die Harnableitung in eine Doppelwindel mit dem Verzicht auf eine Fixierung des Kindes.

Abstract

The therapeutic goal of surgical correction of hypospadias is the reconstruction of the primary malformation into a normal penis. Nowadays the operation is timed around the first birthday of the infant. The procedure is performed in different operative steps depending on the individual components to hypospadias. The current trend is to preserve and tubularize the urethral plate and to stage the procedure in proximal hypospadias with severe chordee. The perioperative management includes modern paediatric anaesthesia methods in combination with regional anaesthesia, urinary drainage into a double diaper and to waive the fixation of the baby at the bed.
