Zusammenfassung
Hintergrund
Single-port-Operationen („laparoendoscopic single-site surgery“, LESS) sind eine neue Methode in der minimalinvasiven laparoskopischen Urologie. Diese mindern das Gewebetrauma für den Patienten, stellen jedoch hohe Anforderungen an den Operateur dar. Wir berichten über unsere ersten klinischen Erfahrungen.
Material und Methoden
Acht Patienten mit unterschiedlichen Pathologien im Abdomen und Retroperitoneum (Nephrektomie, Nierenzystenabtragung) wurden über einen abdominalen LESS-Zugang, zwei Patienten über ein vaginalen NOS-Zugang („natural orifice surgery“) operiert. Zuvor wurden ausgiebige Erfahrungen zur Durchführung und dem Setup im Tierversuch erlangt. Der Zugang erfolgte über verschiedene Single-port-Systeme im Bereich des Bauchnabels.
Ergebnisse
Alle Eingriffe wurden ohne Konversion in ein offen-operatives Verfahren durchgeführt. Zwei zusätzliche Trokare waren bei der ersten Single-port-Operation notwendig, ansonsten war der intra- und postoperative Verlauf bei allen Patienten unauffällig. Das durchschnittliche Alter der Patienten lag bei 58,9 Jahren, die durchschnittliche Operationszeit bei 131 min, der mittlere Blutverlust bei 70 ml. Der mediane Body-Mass-Index (BMI) lag bei 27. In der postoperativ evaluierten Patientenzufriedenheit waren alle Patienten mit dem Gesamtergebnis sehr zufrieden.
Schlussfolgerung
Die Single-port-Chirurgie ist bei entsprechender Erfahrung und Übung eines Teams eine sichere und geeignete Methode zur Durchführung von ausgewählten Niereneingriffen.
Abstract
Background
Single-port surgery (LESS) is a new method of minimally invasive laparoscopic urology. These modern methods reduce the tissue trauma of the patient; however, high demands are placed on the surgeon. We report our initial clinical experience.
Material and methods
Eight patients with different pathologies in the abdomen and retroperitoneum (nephrectomy, renal cyst resection) were treated with an abdominal LESS access and two patients with vaginal NOS (natural orifice surgery) access. Previously, we obtained extensive experience with the setup and implementation in animal studies. The port placements were realized by various single-port systems in the paraumbilical region.
Results
All procedures were performed without conversion to an open surgical procedure. Two additional trocars were needed in the first single-port operation. The intra- and postoperative follow-up was uneventful in all patients. The average age of the patients was 58.9 years, the average operating time 131 min, the mean blood loss 70 ml, and the median body mass index 27. The postoperative evaluation of patient satisfaction revealed that all patients were perfectly satisfied.
Conclusions
With appropriate experience and training of the whole team, single-port surgery is a safe and appropriate method for selected renal surgery.
