Konservative Therapie der postoperativen Harninkontinenz des Mannes

Zusammenfassung

Die männliche Harnkontinenz des Mannes ist meist Folge offener und endoskopischer Operationen an der Prostata. Unmittelbar nach Katheterentfernung sind nach radikaler Prostatektomie bis zu 90% der Patienten inkontinent, ein Jahr danach noch 3–23%. Zur Therapie der männlichen Inkontinenz stehen mehrere Behandlungsoptionen zur Verfügung. Auch wenn die Studienergebnisse zur Wirksamkeit der konservativen Therapiemöglichkeiten teilweise kontrovers diskutiert werden, sollten primär nichtinvasive Maßnahmen versucht werden, bevor ein operatives Verfahren zum Einsatz kommt. Zusammenfassend erscheinen Therapieverfahren wie das Beckenbodentraining mit oder ohne Biofeedback und die pharmakologische Therapie mit Duloxetin effektiv zu sein. Um andere Optionen wie die Elektrostimulationstherapie oder die extrakorporale Magnetfeldstimulation genauer beurteilen zu können, sind weitere randomisierte Studien notwendig.

Conservative management of postoperative urinary incontinence in men

Abstract

Urinary incontinence in men most commonly occurs after radical prostatectomy. Of these patients, 3–23% remain incontinent a year after prostatectomy. Data on conservative therapy for postoperative incontinence is contradictory. Nonetheless, conservative treatment strategies must generally be attempted before any operative technique. Early pelvic floor muscle training with or without biofeedback therapy and duloxetine seem to have a positive effect on continence. Further randomised controlled studies are necessary to accurately assess other conservative therapeutic options such as extracorporeal magnetic innervation and electrical stimulation therapy.
