Zusammenfassung
Die DaVinci-Technologie stellt eine wesentliche Weiterentwicklung der laparoskopischen Operationstechnik dar, bei der der Operateur von einer Konsole aus mit Hilfe einer dreidimensionalen (3D-)Optik laparoskopische Instrumente im Körper des Patienten steuert, die sich in allen Freiheitsgraden bewegen lassen. Dem Operateur wird eine höhere Geschicklichkeit, eine (im Vergleich zur konventionellen Laparoskopie) kürzere Lernkurve, bessere Sichtverhältnisse, eine verkürzte Operationsdauer mit reduziertem Personalaufwand und nicht zuletzt ein ergonomisch optimierter Arbeitsplatz mit minimaler Rücken- und Gelenkbelastung in Aussicht gestellt. Der nachfolgende Artikel beschreibt die Funktionsprinzipien der Technologie sowie die Problematik der vergleichsweise hohen Beschaffungs- und Unterhaltungskosten.
Abstract
The da Vinci Surgical System represents a new technology in advanced laparoscopy: Sitting at a console that allows three-dimensional vision of the operating field, the robotic surgeon is enabled to perform complex laparoscopic procedures. The movements of telemanipulators (“masters”) are transferred to EndoWrist instruments with all degrees of freedom. Compared with conventional laproscopy, the technology allows more dexterity, a shorter learning curve, improved vision, reduced operating time with reduced staff requirements, and an ergonomically designed work station with minimal burden on the back and joints. The following article describes the principles of this technology and the challenges of the capital investment and the running costs.
