Zusammenfassung
Testosteron wird für Effekte an verschiedenen Organen veranwortlich gemacht. Es ist daher plausibel, bei Testosteronmangel dieses Hormon zu substituieren. Anhand von 61 randomisierten Studien ließ sich feststellen: Bei über 65-jährigen Männern kommt es zu einer Zunahme der Knochenmasse (aber nicht zur Abnahme des Frakturrisikos), Zunahme der fettfreien Körpermasse und Zunahme der Erythropoese. Hingegen wurden Veränderungen der kardiovaskulären Parameter, der Serumlipide, der Muskelmasse, der sexuellen Funktionen, der kognitiven Funktionen und der Stimmung nicht eindeutig nachgewiesen. Die Prostata wird nicht nachteilig beeinflusst.
Das Fehlen eindeutiger Beweise für die Gefahr einer Testosteronsubstitution beim alternden Mann bedeutet nicht den Freibrief für eine Therapie ohne Einschränkungen. Solange nicht alle Zweifel ausgeräumt sind, sollten alle Behandlungen sorgfältig dokumentiert und überwacht werden.
Abstract
Testosterone effects different organs. Thus, a substitution of testosterone in the case of hormone deficiency seems plausible. By analysing 61 randomized studies, we demonstrated that in men older than 65 years there is an increase in bone mass (but not a decrease in fracture risk), an increase in lean body mass, and an increase in erythropoiesis. Changes in cardiovascular parameters, serum lipids, muscle mass, sexual functions, cognition and mood, however, are not clearly proven. The prostate is not negatively influenced.
The lack of clear hazards from testosterone substitution in the aging male does not indicate unrestricted treatment safety. Until all doubts are cleared, each treatment should be carefully documented and monitored.
