Akute Meningitis nach transrektaler Prostatapunktion

Zusammenfassung

Bei der durch E. coli hervorgerufenen Meningitis handelt es sich um eine im Erwachsenenalter nur sehr seltene Krankheit. Oftmals sind auch medizinische Prozeduren, wie z. B. Operationen, als Ursache zu finden. Gelegentlich kommt es auch durch die hämatogene Verschleppung des Fäkalkeimes E. coli zu einer akuten Meningitis. Wir berichten über einen Patienten, der nach einer ambulant vorgenommenen Probenentnahme der Prostata an einer E.-coli-Meningitis erkrankte. Ein direkter Erregernachweis gelang sowohl im Liquor als auch in der Blutkultur. Eine breite antibiotische Initialtherapie, die nach Eintreffen des Antibiogramms auf Cefotaxim umgestellt wurde, führte zu einer raschen Besserung des klinischen Bildes des initial komatösen Patienten.

Abstract

Acute meningitis caused by Escherichia coli is a rare disease in adulthood. Medical procedures, e.g. surgical interventions, have been described as a cause. Infection by blood transmission of fecal E. coli is also known. We report a case of acute meningitis after transrectal prostate biopsy. E. coli could be identified both in the cerebrospinal fluid and in the blood culture. A broad initial antibiotic therapy was administered. After cultural isolation of E. coli the therapy was switched to cefotaxime. The initially comatose patient recovered swiftly.
