Zusammenfassung
Die endoluminale Behandlung der Karotisstenose mittels Angioplastie und Stentimplantation ist eine innovative experimentelle Methode, die in den letzten Jahren zunehmend bei Risikopatienten eingesetzt wurde. Bisher fehlen Ergebnisse von validen prospektiven Vergleichsstudien, so dass keine allgemein gültigen Richtlinien für die Indikationen und Technik der Karotis-PTA verfügbar sind. Zahlreiche technische Innovationen zur Vermeidung methodisch-assoziierter Komplikationen finden Eingang in die endovaskuläre Karotisintervention. Dazu zählen, speziell für die Bedürfnisse der A. carotis entwickelte, transfemoral platzierbare Führungskatheter und Stents. Die Miniaturisierung der Stenteinführsysteme lässt eine Reduktion des Passagetraumas auch bei höhergradigen Stenosen und Kinking der Gefäße erwarten. Die mit Hilfe von temporären Okklusionsballons oder Filtern durchgeführte zerebrale Protektion während des Eingriffes hat zu einer Abnahme der neurologischen Komplikationsrate geführt und wird deshalb heute von den führenden Experten für die Karotisintervention als Routinemaßnahme gefordert.
Abstract
Percutaneous transluminal angioplasty and stenting of carotid artery stenosis represents an innovative experimental modality which has been increasingly employed in high-risk patients. Currently, results from prospective randomized trials are not available; thus, guidelines with regard to indications and technique are preliminary. Numerous technical innovations to avoid associated complications have evolved in neurointerventional procedures. Dedicated guiding catheters and stents using a transfemoral approach have been introduced for treatment of carotid artery disease. Low-profile stent delivery systems may decrease risk of releasing embolic load when crossing high-grade stenosis or kinking of the vessel. The introduction of a cerebral protection with the use of temporary occlusion balloons or filter systems has resulted in a reduction of procedure-related neurological complications, and is strongly recommended by leading experts in the field.
