Zusammenfassung
Hintergrund
Akute Blutungen stellen aufgrund des plötzlichen Beginns und der häufig bedrohten Vitalfunktionen eine besondere Herausforderung in der Behandlung dar; ursächlich können z. B. blutende Ulzerationen des Gastrointestinaltrakts oder Tumorblutungen sein. Akute Blutungen können jedoch auch spontan auftreten, traumatisch, iatrogen oder entzündlich bedingt sein.
Zielsetzung
Die Grundprinzipien der Katheterembolisation werden aufgezeigt, sowie verschiedene Embolisationsmaterialien, ihre Eigenschaften und Einsatzmöglichkeiten vorgestellt.
Ergebnisse
Es existiert eine große Bandbreite an verschiedenen Embolisationsmaterialien. Letztlich sind Dauer und Ausmaß der gewünschten Okklusion sowie die Charakteristika des blutenden Gefäßterritoriums die maßgeblichen Faktoren, anhand derer die Auswahl eines Embolisats für eine Anwendung getroffen wird. Es werden mechanische, partikuläre und flüssige Embolisate unterschieden.
Fazit
Unabhängig von der großen Bandbreite der unterschiedlichen Ätiologien ist die radiologische Embolisationstherapie bei computertomographisch nachgewiesener aktiver Blutung eine effektive und sichere Therapieoption, mit der z. T. erheblich präziser, schneller und deutlich schonender als mit den klassischen chirurgischen Verfahren behandelt werden kann. Schlussendlich sind neben technischen Fähigkeiten umfassende Kenntnisse des Interventionalisten über die entsprechenden Charakteristika der verschiedenen Embolisationsmaterialien für den Therapieerfolg entscheidend.
Abstract
Background
Treatment of acute hemorrhage is particularly challenging due to its sudden onset and the fact that it is often life-threatening. Potential causes include bleeding ulcerations of the gastrointestinal tract or tumor bleeding, but acute bleeding can also occur spontaneously or be traumatic, iatrogenic, or inflammatory.
Objective
To demonstrate the basic principles of catheter embolization, as well as various embolic materials and their properties.
Results
Today there are a wide range of embolization materials available. Ultimately the duration and extent of the desired occlusion are decisive when selecting an embolic material. Mechanical occlusion devices, particulates, and liquid embolic agents are available.
Conclusion
Irrespective of the wide range of different etiologies, radiological embolization therapy is a very safe treatment option in cases of computed tomographic evidence of active hemorrhage. Ultimately, the interventionalist’s comprehensive knowledge of the relevant characteristics is crucial for the success of therapy.
