Zusammenfassung
In dieser Arbeit sollen die Grundsätze der interdisziplinären Behandlung von Hirnmetastasen skizziert werden. Die interdisziplinäre Behandlung wird bestimmt durch die klinische Situation, anatomische Gegebenheiten und die Tumorentität. Sie hat eine Reduktion der Toxizität im Fokus. Zur Diagnostik von Hirnmetastasen werden Magnetresonanztomographie, Computertomographie (CT) und Positronenemissionstomographie-CT verwendet. Bei zugänglichen, symptomatischen Metastasen wird die Neurochirurgie eingesetzt. Bei umschriebenen, auch multiplen, jedoch chirurgisch schlecht erreichbaren Metastasen kommt die Radiochirurgie zum Einsatz. Die Teilhirnbestrahlung wird, wenn möglich, einer Ganzhirnbestrahlung vorgezogen. Eine Schonung des Hippocampus bei Ganzhirnbestrahlung reduziert die Therapietoxizität. In Notsituationen leisten Steroide einen effektiven Beitrag, eine neurochirurgische Intervention ist eventuell lebensrettend. Die Möglichkeiten einer medikamentösen Systemtherapie nehmen zu.
Abstract
The aim of this work is to outline the principles of interdisciplinary treatment of brain metastases. Interdisciplinary treatment is determined by the clinical situation, anatomical conditions and tumor entity and has the goal of reducing toxicity. Magnetic resonance imaging, computed tomography (CT) and positron emission tomography-CT are used to diagnose brain metastases. Neurosurgery is used for accessible, symptomatic metastases. For localized metastases, including multiple metastases, that are surgically inaccessible, radiosurgery is used. If possible, partial brain irradiation is preferred to whole-brain irradiation. Protection of the hippocampus during whole-brain radiotherapy reduces therapy toxicity. In emergency situations, steroids provide effective support and a neurosurgical intervention may be life-saving. The options for systemic drug therapy are increasing.

Bei einer Diagnose von Hirnmetastasen werden nur fluorierte Kortikosteroide appliziert.
Unmittelbar nach der Diagnose von Hirnmetastasen werden in jedem Fall Antiepileptika und Kortikosteroide als Dauermedikation verabreicht.
Es werden prinzipiell Steroide und Relaxanzien verordnet.
Bei Vorliegen einer neurologischen Symptomatik aufgrund des Tumorödems wird eine Gabe von Kortikosteroiden initiiert. Bei Auftreten von einfach fokalen oder generalisierten epileptischen Anfällen erfolgt eine antikonvulsive Therapie.
