Zusammenfassung
Klinisches/methodisches Problem
Hirntumoren sind im Kindesalter die häufigsten soliden Tumorerkrankungen und nach der Leukämie die häufigste Krebserkrankung. Die Inzidenz nimmt kontinuierlich zu. Die seit 2016 gültige WHO-Klassifikation der Hirntumoren basiert jetzt auf der Kombination histologischer und molekulargenetischer Diagnostik.
Radiologische Standardverfahren
Die Diagnostik erfolgt überwiegend mittels Magnetresonanztomographie (MRT); nur im Notfall wird eine Computertomographie (CT) durchgeführt.
Methodische Innovationen
Es kommen diffusions- und suszeptibilitätsgewichtete sowie dynamische kontrastverstärkte Bildgebungsmethoden sowie die Spektroskopie zum Einsatz.
Leistungsfähigkeit
Durch die angewandten Bildgebungsverfahren kann eine Verbesserung der Art- und Dignitätsbestimmung, Größenbestimmung, Beurteilung der Nachbarschaftsbeziehung und morphologischen Beschreibung der Tumorzusammensetzung erreicht werden.
Bewertung
Die moderne MRT mit funktionellen Techniken stellt heute den Goldstandard für die Differenzialdiagnose und das Staging von Tumoren des Zentralnervensystems (ZNS) im Kindesalter dar.
Abstract
Clinical/methodological issue
Brain tumors are the most common solid tumors in childhood and the most frequent cancer after leukemia. The incidence is continuously increasing. The WHO classification of brain tumors, valid since 2016, is now based on the combination of histological and molecular genetic diagnostics.
Standard radiological methods
Diagnostics are mainly performed with magnetic resonance imaging (MRI); only in emergencies with computed tomography (CT).
Methodological innovations
Diffusion and susceptibility weighted and dynamic contrast-enhanced imaging and spectroscopy are used.
Performance
Improved diagnosis regarding dignity, size determination, adjacency assessment, and morphological description of tumor composition.
Achievements
Modern MRI with functional techniques is now the gold standard for differential diagnosis and staging of central nervous system (CNS) tumors in pediatrics.
