Okklusion der V. cava superior

Zusammenfassung

Wir berichten über eine junge Patientin mit Cancer-of-unknown-primary(CUP)-Syndrom, bei der infolge einer ausgeprägten mediastinalen Lymphadenopathie eine Thrombosierung und Okklusion der V. cava superior vorlag. Zur Fortführung der palliativen Chemotherapie wurde ihr perkutan ein zentralvenöses Miniportsystem über einen rechtsfemoralen Zugangsweg implantiert. Das Portreservoir wurde kraniomedial der rechten Spina iliaca anterior superior platziert. Dieses Vorgehen erschien im Hinblick auf den Tragekomfort von Hosen günstig. Zudem ist an dieser Stelle das Portreservoir für das medizinische Personal relativ komfortabel zugänglich. Über das Portsystem konnte sowohl Blut aspiriert als auch komplikationslos die Chemotherapie appliziert werden. Der Fall zeigt, dass bei Okklusion der V. cava superior die perkutane transfemorale Einbringung eines zentralvenösen Portsystems eine hilfreiche Option darstellt.

Abstract

We report on a young patient with cancer of unknown primary origin with occlusion of the superior vena cava due to mediastinal lymphadenopathy. In order to continue infusion of palliative chemotherapy a central venous port system was radiologically implanted via a right femoral vein access. The port reservoir was placed craniomedial to the right anterior superior iliac spine. This approach was considered convenient in respect to comfort in wearing trousers. In addition, at this site the port reservoir is easily accessible for medical staff. It was possible to draw blood via the port system and to infuse the chemotherapy without complications. The present case shows that in cases of occlusion of the superior vena cava radiological implantation of a central venous port system via a femoral vein access is a useful option.
