Zusammenfassung
Die aktuelle Typisierung und Gradierung diffuser Gliome beruht ausschließlich auf subjektiven morphologischen Parametern, und die TNM-Klassifikation hat für Hirntumoren keinen Stellenwert. Hier werden zusätzliche Parameter vorgestellt, die sich für eine weitergehende Klassifikation und Dokumentation eignen.
Als zusätzliche Parameter zum WHO-Grad diskutieren wir Altersgruppen, unterschiedliche Verlaufsformen, Begleitumstände wie Zweitneoplasien oder familiäre Tumorleiden, Tumorausdehnung auf anatomisch definierte Hirnregionen und deren Anzahl, den Karnofsky Performance Scale, Eloquenz der betroffenen Hirnregionen, Diagnosesicherheit und Repräsentativität von Gewebeproben.
Die hier vorgestellte Klassifikation und Dokumentation tragen dazu bei, insbesondere auch langjährige komplexe Krankheitsverläufe von Gliompatienten besser zu verstehen, zu überblicken, besser elektronisch erfassbar und für weitergehende Studien besser vergleichbar zu machen. Schließlich zeigt diese Arbeit auch, dass eine Gradierung und eine Klassifikation nicht ohne Kenntnis klinischer und radiologischer Befunde erfolgen sollten.
Abstract
Most current grading systems of diffuse gliomas are based solely on the microscopic evaluation of surgical specimens and the TNM classification does not have a value for brain tumors. Here additional parameters are presented, which are suitable for a classification and documentation of diffuse gliomas.
As additional parameters to the WHO typing and grading we discuss age groups, different tumor devolutions, circumstances such as a second malignant neoplasm or hereditary tumors, tumor expansion based on anatomically defined brain regions, Karnofsky Scale, eloquence of the brain regions, diagnostic certainty and informativity of tissue samples.
This work shows that clinical data and imaging studies can contribute substantially to the classification of diffuse gliomas. The additional parameters presented here constitute a significant improvement of glioma documentation. Especially complex courses of long duration and repeated therapeutic interventions can be better surveyed and digitally processed.
