Zusammenfassung
Der Zielwert der mittleren optischen Dichte für Mammogramme einschließlich adäquater Toleranzen wird im “European Protocol for the Quality of the Physical and Technical Aspects of Mammography Screening (EPMS)” [1] festgelegt und ist unabhängig vom Screening sinnvoll. An 22 Mammographieeinrichtungen wurden Prüfkörperaufnahmen mit unterschiedlicher kV und PMMA-Dicke angefertigt und es wurden Sensitometerstreifen zur Überprüfung des Zustandes der Filmverarbeitung angefertigt. Nur 5 von 22 Einrichtungen halten alle Anforderungen des EPMS an die optische Dichte (o. D.) ein. Im Wesentlichen gibt es dafür 3 Ursachen: 1) falsche Festlegung der optischen Dichte unter Standardbetriebsbedingungen, 2) falsche Kompensation der Expositionsbedingungen bei variierender Röntgenröhrenspannung (kV) und PMMA-Dicke durch die Belichtungsautomatik sowie Fehlanpassungen der Schrittweite für die Korrekturtaste und 3) mangelnde Berücksichtigung des Zustandes der Filmverarbeitung. Alle untersuchten Einrichtungen zeigen keine Auffälligkeiten oder Beanstandungen in der Konstanzprüfung nach DIN 6868-2 [22] und DIN 6868-7 [3]. Die auffälligen Einrichtungen sind offensichtlich in der Abnahmeprüfung nach DIN 6868-52 (1990-12) [4] nicht sachgerecht justiert worden bzw. in den Prüfungen wurde gegen Grenzwerte geprüft, die den Anforderungen an die optische Dichte in Mammogrammen nicht angemessen sind.
Abstract
The requirements for the target value of the mean optical density (o. d.) including proper tolerances of mammogrammes are determined in the “European Protocol for the Quality of the Physical and Technical Aspects of Mammography Screening (EPMS)”. These requirements are not only applicable to screening mammography. 22 mammography devices were tested with exposures of test phantoms using different kV and thickness of PMMA and with sensitometric step wedges to check the processing conditiones. Only five of 22 devices fullfill all requirements of EPMS to the opt. density. These results are caused by three reasons: first, wrong locking of optical density under standard exposure conditions; second, wrong compensation of the optical density by the automatic exposure controll due to variing tube voltage (kV) and PMMA thickness and third, an inadequate back coupling to the processing conditions. All tested devices are not conspicuous in respect to the results of constancy tests according DIN 6868-2 [2] and DIN 6868-7 [3]. It is obvious, that the claimed devices are not properly adjusted in the acceptance test according DIN 6868-52 (1990-12) [4]. Respectively the tolerances, used do not fullfill the reqirements to proper density in mammogrammes.
