Zusammenfassung
Bei der individuellen Beratung über die Notwendigkeit einer Malariaprophylaxe muss man zwischen dem Risiko an einer Malaria zu erkranken oder gar daran zu versterben und dem Risiko potenzieller Nebenwirkungen abwägen. Bei Epilepsiekranken muss zusätzlich das Risiko abgeschätzt werden, durch die Prophylaxe einen epileptischen Anfall zu erleiden. Prokonvulsive Effekte sind nachgewiesen beim Mefloquin. Die Erfahrungen bei Epilepsiekranken mit neueren Medikamenten wie z. B. dem Atovaquone-Proguanil sind unzureichend. Ausgehend von einer Risikoabwägung werden Vorschläge zur Malariaprophylaxe bei Epilepsiekranken gegeben.
Summary
Recommendations for malaria prophylaxis must balance the risk of malaria against the risk of side effects of malaria prophylaxis. In patients with epilepsy, the risk of inducing epileptic fits must also be considered. Mefloquine has been shown to have proconvulsive effects and induce seizures. For other drugs like atovaquone-proguanil, the experience up to now is insufficient in epileptic patients. Considering this balance of risk vs benefit, recommendations are given for malaria prophylaxis in patients with epilepsy.
