Hintergrund

Angststörungen gehören mit einer 12-Monats-Prävalenz von 14 % zu den häufigsten psychischen Erkrankungen in Europa und gehen mit einer hohen sozioökonomischen Belastung einher. Die Entstehung von Angststörungen ist komplex mit einem Zusammenwirken von biologischen, umweltbedingten und psychosozialen Risikofaktoren, die in unterschiedlicher individueller Konstellation im Sinne eines Vulnerabilitäts-Stress-Modells das Gesamterkrankungsrisiko bedingen.

Material und Methoden

In dieser Übersichtsarbeit werden Risikofaktoren für Angststörungen auf verschiedenen Ebenen – beginnend mit der Genetik, Umweltfaktoren, Gen-Umwelt-Interaktionen und der intermediären Ebene der Epigenetik über neuronale Netzwerke („brain fear circuit“) und psychophysiologische Faktoren (z. B. Schreckreflex, CO2-Sensitivität) bis hin zu dimensionalen klinischen Phänotypen wie Angstsensitivität und „behavioral inhibition“ – vorgestellt und hinsichtlich ihres ggf. nutzbaren prädiktiven Werts diskutiert.

Ergebnisse und Diskussion

Zum jetzigen Zeitpunkt ist der prädiktive Wert der genannten Risikofaktoren im Rahmen eines probabilistischen Prädiktionsmodells nicht sicher zu beurteilen. Die Identifikation von Risikofaktoren für Angststörungen ermöglicht jedoch bereits jetzt frühzeitige Angebote zur Prävention von Angststörungen und kann damit zur Reduktion des mit der Erkrankung verbundenen persönlichen Leids sowie einer Kostenersparnis im Gesundheitssystem beitragen.

Summary

Background

Anxiety disorders are among the most frequent mental disorders in Europe (12-month prevalence 14 %) and impose a high socioeconomic burden. The pathogenesis of anxiety disorders is complex with an interaction of biological, environmental and psychosocial factors contributing to the overall disease risk (diathesis-stress model).

Material and methods

In this article, risk factors for anxiety disorders will be presented on several levels, e.g. genetic factors, environmental factors, gene-environment interactions, epigenetic mechanisms, neuronal networks (“brain fear circuit”), psychophysiological factors (e.g. startle response and CO2 sensitivity) and dimensional/subclinical phenotypes of anxiety (e.g. anxiety sensitivity and behavioral inhibition), and critically discussed regarding their potential predictive value.

Results and discussion

The identification of factors predictive of anxiety disorders will possibly allow for effective preventive measures or early treatment interventions, respectively, and reduce the individual patient’s suffering as well as the overall socioeconomic burden of anxiety disorders.
