Zusammenfassung
In der Akuttherapie von Schlaganfallpatienten konnten in den letzten Jahren erhebliche Fortschritte erreicht werden. Patienten, die dabei einen schweren Schlaganfall in der Akutphase überleben, bleiben oft in ihren motorischen, kognitiven und kommunikativen Fertigkeiten hochgradig eingeschränkt. Wenn bei solchen Kranken der Tod absehbar ist, sollten kurative Therapiemaßnahmen konsequent unterbleiben. Stattdessen müssen die Behandlungsziele palliativmedizinisch definiert und auf die Linderung quälender Symptome konzentriert werden. Dabei sind palliativmedizinische Konzepte in der Neurologie mittlerweile gut etabliert, z. B. bei der amyotrophen Lateralsklerose. Gerade die Schlaganfallpatienten, welche sich mit massiven neurologischen Ausfällen über längere Zeiträume stabilisieren, bedürfen ebenfalls einer interdisziplinären palliativen Betreuung, bei der über die Symptomlinderung hinaus reflektiert werden muss, wie auf eventuelle medizinische Komplikationen reagiert werden soll und ob lebenserhaltende Maßnahmen fortgesetzt werden dürfen. Als Grundlage dieser Überlegungen sollte zunächst die medizinische Indikation und daraufhin der erklärte oder mutmaßliche Patientenwillen herangezogen werden.
In der vorliegenden Übersicht werden die palliativmedizinischen Kernprobleme und symptomorientierte palliativmedizinische Behandlungskonzepte für Patienten mit schweren Schlaganfällen dargestellt.
Summary
Recent years have seen considerable advances in acute stroke treatment. Patients who survive the acute phase following major stroke often retain severe limitations of motor, cognitive, and communicative functions. In such patients whose death is imminent, curative therapies should be avoided. Instead, the goal of any therapy should shift to palliative treatment and concentrate on reducing causes of suffering. Palliative methodologies and concepts are already well established in neurology, e.g. for amytrophic lateral sclerosis. Particularly those stroke patients who remain stable for a long period with massive neurological deficits require interdisciplinary palliative care that goes beyond mere reduction of symptoms and seeks a response to possible complications and the question of maintaining life-preserving measures. Not only medical indications but also the expressed or presumed will of the patient must be included in such considerations. This article describes and elucidates basic questions of palliative medicine and symptomatic palliative treatment concepts for stroke patients.
